Good Evening:
As a courtesy to all teachers who request me as a substitute, I find it very helpful to the teacher
and to their future planning if accurate feedback is provided as to students behavior and our
level of academic productivity.
Below you will find the substitute report sheet that I have created and filled out for your records.
Please feel free to forward this report to any administrator in your building should you find it in
any way advantageous.
I wish to extend to you my sincere thanks for allowing me the distinct opportunity to serve you
and your students. It was an insightful and gratifying experience; I only hope I can return to your
classroom in the future and once again, continue to be an integral part of your students lives.

Same as period #1. Period #4 had your overhead projector cut out yesterday and Tuesday just
about 1 minute before each presentation, so I wasnt sure if you wanted to finish showing the
rest. However, know that they had all the material written down for both writing assignments.

someone left their wheel and dowel to their gumball machine on the table on Monday. No one
knew who it belonged to, so I will try my best to touch base with your sub today to remind them

Rami was whining a bit todaybehavior very unbecoming. His constant complaint was that he
didnt want to learn coding. I emphasized to the entire class what a wonderful skill it was to have
given the digital age we are living in. Donald was just a mess this period. Twice this period he
was caught on the computer in the computer lab playing games while we were in the middle of
our screen cast. Then we wouldnt sit still in his chair. The next thing I know, he fell on the floor.
He claims he leaned back in the chair and fell.
