Dear Mr. Lassiter,

I am writing in regard to your allegation of May 18, 2010. You stated that Bethany
Christian Services staff members violated your right to confidentiality by releasing your
full name to the adoptive parents of your infant daughter without your consent. An
investigation was initiated in which involved staff members were interviewed and
relevant documents were reviewed. Your allegation was substantiated.

My investigation found that the Adoptive Family worker failed to check for a signed
release from you before he gave court documents to the adoptive family. The adoptive
family saw your full name on the court documents when they signed them. Although the
oversight was inadvertent, it did violate your right to confidentiality as well as Bethany
Christian Services policy. This was not a HIPAA violation as no protected health
information was disclosed.

I want to assure you that corrective action has been taken with the responsible worker. In
addition, our procedure has been changed to better protect the confidentiality of birth
parents. Also, as a result of your complaint, all infant adoption staff members received
additional training on better protecting the rights of birth fathers.

On behalf of Bethany Christian Services, I want to apologize for this oversight. I also
want to thank you for bringing this situation to our attention. We appreciate the
opportunity you gave us to improve our services to clients. It is always our goal to
provide caring, high quality adoptive services that respect the rights of all parties
involved.

If the investigative findings and action plan are not satisfactory to you, you may file a
written appeal within 14 days .You may direct your appeal to the Michigan Director of
Operations, Mr. Brian De Vos. At your request, a conference will be granted in person or
by telephone. Mr. De Vos, in consultation with our Chief Operating Officer and/or
President, will arrive at a final decision within 15 business days. Again, thank you for
bringing our attention to this matter.
