    1. Pre-Defined Template/Checklist for CL/RE Requests

    2. Order status check for CL/RE using the New Order

    Cause  Duplication - Macro got crashed on order generation


     Macro was executed again without prior checks on
    any order booking in DOMS

    Total Impact 29 orders/ 29 units - $ 12,953.70

    Corrective Action

    • Educated the team to ensure any orders placed in DOMS on Macro crash
    • Instructed to check the output sheet any order number got reflected
    • Edit the input sheet as by the output sheet rows prior to run the macro.
    • 100 % validation implemented to prevent the duplication.

    ISR placed CLRE request due to LCD issue
    Request was uploaded in POM.
    Orders must be cancelled prior to rebook.
    Total Impact 6 orders – 1660 units - $ 1,245,000

    Corrective Action
    Three Level checks implemented.
    • Order cancellation would be ensured before request upload on POM by the Gate
    Keeper
    • OE Team was instructed to check about order cancellation prior to rebook
    • CL/RE check point is included the Validation tracker to ensure for no duplication

     Over shipped 540 Boxes – customer needed 18 units.
    Cause
     Input sheet prepared with 18 x 18 instead of 1 x 18.
     270 systems were dispatched instead of 18.
    Total Impact 252 orders - $160,020

    Corrective Action 100 % validation implemented to ensure 100% quality
