PROVISIONAL 2D-ECHO AND DOPPLER

The left ventricle is dilated with mild global hypokinesia with EF of 48 % by


Teichholz’s and 43 % by Simpson’s.
The left ventricular muscle mass is 240 g on AL, 253 g on diastole, 274 g on
systole.
The left atrium is dilated normal in size. The LA volume is 108 cm3 and the
LAVI 63 cm3/ m2.
The right atrium and right ventricle is dilated. There is note of flattening of
the interventricular septum during systole and diastole indicative of right
ventricular pressure and volume overload. TAPSE 1.10
The anterior and posterior mitral valve leaflets are thickened without
restriction in opening. The mitral valve annulus measures 6.3 cms by PLAX,
4.7 cms by 4-C.
The tricuspid and pulmonic valves are thickened .The tricuspid valve annulus
measures 5.5 cms.
The right coronary and non-coronary aortic cusps are thickened.
The interventricular and interatrial septa are intact.
There is no thrombus nor vegetation seen.
No pericardial abnormalities noted.

On color-flow Doppler:

There is an eccentric mosaic color flow across the mitral valve during systole
with an RF of 71% consistent with severe mitral regurgitation.
There is a mosaic color flow across the tricuspid valve during systole with an
RF of 52% consistent with severe tricuspid regurgitation
There is no mosaic color flow across the aortic valve.
There is mosaic color flow across the pulmonic valve during diastole
consistent with pulmonic regurgitation.
The computed PAP is 55 mm Hg by TR jet and 96 mm Hg by PAT indicating
moderate to severe pulmonary hypertension.

Conclusion:

Dilated left ventricle with mild global hypokinesia with mildly


depressed overall systolic function
Dilated right atrium and right ventricle with signs of pressure and
volume overload.
Mitral, Tricuspid and Aortic valve sclerosis
Sever mitral and tricuspid regurgitation
Pulmonic regurgitation
Moderate to Severe pulmonary hypertension
