PROCEDURE FOR OBTAINING THE PAN

1. We are attaching herewith, BLANK PAN APPLICATION FORM 49A (in PDF format),
which you will download and get it, printed.

2. You will not fill-up FORM 49A.

3. You are required to sign at specified two places in Form 49A (mentioned below)
in BLACK INK only in the boxes provided for signature (sign should not go
outside the box otherwise application will not be accepted).

a. On the top right hand corner of the Application form below the photograph
on page 1.
b. On the backside of the Application form on page 2, at the bottom of the
Right hand corner.

4. We are attaching herewith PAN Data Form (Excel Sheet) in which you are
required to furnish us details.

5. Based on this information we shall prepare the application for obtaining


Permanent Account Number.

6. You shall send the following documents through courier / post.

a. Signed PAN Application Form 49A;


b. Two passport size photograph (size of photograph has to be “3.5 cm & 2.5
cm”);
c. The Passport copy should be duly certified by the Indian Embassy Abroad
as the proof of the Identity and Residence

In case the address mentioned in your passport or driving license is other


than the one you like to provide as Residence address for PAN application,
then you may provide either of the following documents having that address:
 Copy of Bank Statement.
 Copy of any Utility bills.
 Copy of Passport.

7. We are also attaching a PAN Correction form herewith. You are required to sign
the PAN correction form on two places one is top right hand corner below the
space provided for photograph and other is bottom right hand corner.

We are taking your signature on this form because if there is some mistake or
error on the Pan Card issued as to name, father’s name, date of birth, or
photograph then we can rectify the same by making application through this
form. This will save your and our time.

You shall receive the PAN card directly at your address of communication. Also, for your
information, we shall put the address of our office as care off for the purpose of delivery
of the card, which shall then be sent to you by courier as and when received. The card
normally takes 15-25 days from the date of its application to the PAN processing
department.

Also once the card is received at our end we shall send you the scanned copy for your
verification and approval. On your confirmation we shall then courier the card at the
address given in the PAN Data form.

OUR FEES.

Our charges shall be Rs.2020/- (i.e.Rs.1000 per application, Rs. 800 courier charges plus
12.36%service tax), if the card is to be delivered outside India and Rs.1124/- (i.e.Rs.1000 plus
12.36%service tax), if the card is to be delivered within India.
In case you are sending a Demand Draft or a Cheque (If you have a bank account in India) -

It should be drawn in favour of "Yeshwant Mehta" payable at Mumbai and sent by courier to the
