Affidavit of Support

This form must be completed by student sponsor. If student is requesting admission as an

international student based on his/her own funds, student must complete both the
applicant as well as the sponsor section on this form. Official financial statements (e.g.
bank statements) must accompany affidavit of support. Statement of support must not be
more than three months old from the date of application.
