Statewide Community Pandemic Influenza Outreach and Community Education

January 7, 2011. The Illinois Department of Public Health’s office is closed on December 31, 2010.
The Illinois Department of Public Health, Office of Preparedness and Response, Division of Disaster Planning
and Readiness, is making available unspent CDC Public Health Emergency Response (PHER) grant funding.
The purpose of the funding is to continue previously approved PHER-funded activities, or conduct new
activities that address or retest identified gaps in local health department pandemic flu response. The funding is
designated for Statewide Community Pandemic Influenza Outreach and Community Education programs.
Priority consideration will be given to proposals that target one or more of the following populations including
not limited to: disability community, limited English language proficient communities, senior citizens, rural and
communities of color. Eligible applicants include, but are not limited to: community-based organizations, non-
profits, private associations, local health departments, faith-based organizations, voluntary organizations,
organizations serving youth, organizations serving ethnic populations, schools/school districts, and any entity
collaborating with local or state governments in this effort.

• Applicants who can show coordination and collaboration with Local Health Departments and other
eligible organizations will be awarded preference. This coordination will eliminate overlapping or
conflicting efforts in regions.

• The requested scope of work in submitted proposals should clearly indicate: the identified gap in the
pandemic response, the targeted group, the program that will be implemented to educate and reach the
targeted group (goals and objectives), description of the evaluation tool that will be utilized to measure
success and a description on how the proposed program can be a model that can be used in the future.

• The cost of vaccinations is a prohibited expense for this funding.

• All proposals will be submitted through the Illinois Department of Public Health grant review process.

• Funding is limited to service areas outside of the city of Chicago.
