Standard Operative Procedures (SOPs) – Pediatric Medical OPD

1. The ‘Triage’ nurse should be vigilant and identify the serious/unstable children in
the OPD and expedite their care on priority basis. Such patients should be
immediately transferred to the A&E department/ward accompanied by the doctor
on duty.

2. The stable patients will be initially seen in the medical filter clinics according to
their registration order. In complicated/problem cases the specialist on call will do
the assessment and decide the appropriate management.

3. There must be proper documentation of patient’s profile in the OPD register.


Write down the name, age, gender, hospital number and diagnosis. Mention any
referral or admission.

4. The patients should be instructed regarding the treatment (dose and duration) and
need for follow-up. Reassure them if no medicine is required.

5. The prescriptions must be in legible writing indicating patient’s name, hospital


number, date and strength of the drug. The drug should be written in capital
letters. It should be duly signed and stamped. All doctors should own a stamp
with name and designation. In case of non-availability of stamp, the doctor must
put his/her name in capital letters below his/her signatures.

6. As the bed strength in the wards is limited, the patients with severe acute illnesses
shall be given priority for admission over those with chronic disorders who may
need admission just for diagnostic evaluation.

7. Admission form must be filled in accurately indicating patient’s name, hospital


number, date & time of admission, name of admitting specialist, clinical
condition, provisional diagnosis, and initial management. It should be properly
signed and stamped by the admitting doctor.

8. After the initial assessment in the medical filter clinics, the patients may be
referred to the respective sub-specialties for ongoing care.

9. When a patient needs consultation from another OPD clinic, he/she should be
seen on priority basis with no more wait in line. The referring doctor should
inform the concerned doctor on phone and/or send the patient accompanied by the
peon/aya.

10. The un-immunized/partially immunized children should be counseled and referred


to the immunization department to complete the vaccine doses.

11. Mothers of young children with unsatisfactory/problematic breast feeding practice
should be referred to the lactation clinic for counseling and education.

12. The children with poor nutritional history and growth failure need to be referred
to the growth monitoring room for counseling and follow-up.

13. All the units should display the duty-roster of doctors in the follow-up clinic. The
doctors should stay in the follow-up clinic for the specified time.

14. The day to day problems in the OPD should be tackled by the senior registrar/ In-
charge OPD/Specialist on call. They must ensure that the hospital staff follows
the SOPs in letter and spirit.

15. Unit on call is responsible to augment the medical staff in the OPD as and when
required.

16. Growth charts (Weight, Height, and head circumference) and Blood pressure
apparatus must be available in each clinic.

17. The pharmacy should provide an updated list of available drugs in each OPD
clinic.

18. The OPD staff must be punctual. No one should go on leave without prior
information and permission. Doctors in a given OPD clinic should go for tea in
turn. No OPD clinic should be left unattended at any given time.

19. The doctors should wear white coat and display their hospital identity card.

OPD (CH) Timings

All working days (except Friday): 08:00 to 14:00 hrs.


Friday: 08:00 to 12:00 hrs.

The registration of new patients is done from 8.00 to 12:30 hrs.

OPD (CH) Setup

General medicine: Room No. 6, 7, 8, 9 and 10.

Medical follow-up clinic: Room No. 3.


Unit-I = Tuesday, Thursday, Saturday.
Unit-II = Monday, Wednesday, Thursday

General surgery: Room No. 4 and 5.

Neonatology:
Room No.

Specialist Clinics:
Room No.
Nephrology: Tuesday
Neurology: Wednesday
Endocrinology: Thursday
Pediatric Infectious diseases: Monday, Wednesday
