Performer shall render artistic services in connection with the project(s) designated below (the
“Project”), as follows:

TITLE OF PROJECT: NAME

PLACE OF PERFORMANCE: New York, NY or as otherwise required by Producer.

TERM OF SERVICES: Full day of performer services on DATE Additional days as needed and
agreed upon.

DESCRIPTION OF SERVICES: Performer shall render all necessary and customary dramatic
performance services as required by Producer.

COMPENSATION: In consideration of Performer’s complete and timely performance of his

obligations hereunder and the rights herein granted, and provided Performer is not in breach of this
Agreement, Producer shall pay to Performer the “all-in”fee of $FEE

Producer’s Standard Terms and Conditions (the “Standard Terms”) attached hereto are by this
reference incorporated herein and made a part of this agreement (“Agreement”). Without limiting the
generality of the foregoing, and notwithstanding anything to the contrary set forth in this Agreement,
all of Producer’s and Performer’s rights and obligations are subject to the Standard Terms. In the
event of any conflict between the foregoing provisions of this Agreement and the applicable provisions
of the Standard Terms, the foregoing provision(s) of this Agreement will govern.
Execution of this agreement signifies acceptance by Producer and Performer of all of the terms set
forth above and all of the Standard Terms and Conditions which are attached hereto and incorporated
herein by this reference.

CONSENT OF PARENT OR GUARDIAN (IF UNDER AGE 18)

The undersigned, as the parent/guardian having legal custody of the above-named Performer, hereby gives permission for
Performer to enter into this Agreement. I have read and fully understand this Agreement. I affirm all representations and
warranties made in this Agreement and guarantee the performance of this Agreement by Performer and will not revoke my
consent.
