Thank you for your letter of June 23 with the accreditation information.

As you requested I include here your form duly filled in (by hand).

You will find in the form the names of the persons I indicate for commendation. Revd. Brent Fisher
has already sent to Pat Took his commendation for me, but you might ask him again if you need it.

I wrote to the Baptist church of Cuneo for the statements you need for my membership there. I will
forward it to you as soon as they respond. My membership there goes back to the seventies, before I
moved to Switzerland for my theological studies and my pastorate. In the Swiss Reformed church
where I was a pastor, you are a church member if you live there, declare so and pay the appropriate
church taxes.

I include here my current CV (which I could further integrate with more information if you need it),
and photocopies of my Baptism Certificate, my Bachelor of Divinity Certificate, and my Ordination
Certificate as a Pastor. I also include a copy of my Italian passport, declaring my EU citizenship
and, consequently, my right to work in the UK. I have a British National Insurance number.
