Dear Sir, With due respect, I am pleased to inform you that I was serving as a lecture position in Management in the Department of business administration 7/09/2010 to 1/05/11 and resigning from that position was 02/05/2011 for that reason I want to withdrawal my provided fund money from your institution .But my physical sickness I am not able to collect it by myself .So I am given to my authorization power for withdrawal my provided fund money from Northern University by my Husband Mohammad Abdullah Mahfuz (Who work as a Senior Lecturer in Northern University Bangladesh in Marketing department ).
