JOINT-AFFIDAVIT OF COHABITATION We, Benjamin A. Bualat Jr. of legal age, Filipino, Ivy Joy G. Ombayan, of legal age, Filipino, both are residents of #14 Emerald St. Ecoland Subdivision, after having been sworn to in accordance with law hereby depose and say that: 1. We became sweethearts on the year 2006; 2. On the same year specifically on November 28, 2006, we started to live and treat each other as husband and wife living in one roof up to the present without the benefit of marriage; 3. During our cohabitation, we do not engaged in other relationship with opposite sex; 4. Our cohabitation is continuous and uninterrupted from November 2006 up to the present which is more than five (5) years; 5. We have two children namely: Louie Jay O. Bualat and Daryn Nicole O. Bualat. Louie Jay is three (3) years old and Daryn Nicole is one (1) year old; 6. We planned to get married on January 2012 for our interest and for our children as well so that they will be legitimated; 7. We are executing this affidavit to attest the truth of the foregoing and to inform the properties concern to exempt us from obtaining marriage license.
