All submissions must be bound or stapled in the top left-hand corner and placed in a plastic envelope with this cover page. Do not use single page plastic sheet display folders. In cases where information must be submitted on a diskette or CD: * Enclose the diskette/CD with this cover page and any other documentation to be submitted in a plastic sleeve or, for large or bound submissions enclose the disk and documentation in a suitable folder to prevent loss. * * * * Store only one assessment event per diskette/CD. No other work is to be stored on the diskette. Use only one diskette/CD per submission (space permitting). Label all diskette/CDs with the subject number, assessment event identification and student name(s). Use appropriate file names to facilitate identification of items.
