Disturbed sleep pattern related to environment temperature and changes in sleep

surroundings. P I : After 6 hours of nursing interventions, the patient will report improved sleep pattern. : Established rapport with patient. Monitored vital signs every 4 hours. Provided comfort to the patient such as bedside care to promote sleep. Provided quiet environment to enhance relaxation. Provided blanket to control the temperature of the patient. Provided safety by raising the side rails up and lower the height of bed. E : After 6 hours of nursing interventions, the patient reported improved sleep pattern.

: No subjective cues. : Patient received lying on bed Patient is awake, conscious, and coherent. Vital signs taken: BP: 140/90 With ongoing IVF D5LR 1L x 8 at right hand. With wound dressing dry and intact. With post operative incision at mid of the abdomen.

: Impaired tissue integrity related to post operative incision at mid of the abdomen. : After 8 hours of nursing interventions, the patients wound will be free from any infection.

: Established rapport with patient. Monitored vital signs every 4 hours. Performed bedside care to promote comfort and prevent accumulation of bacteria. Performed hand washing or sanitized hands before and after contact with patient. Provided patient a good hygiene to prevent any infection and increase comfort. Provided safety by raising the side rails and lowering the height of bed. Provided a quiet environment to promote comfort.

: After 8 hours of nursing interventions, the patients wound was free from any infection.

: Acute Pain related to post operative incision at mid of the abdomen. : After 1 hour of nursing interventions, the patients pain will be reduced or controlled. : Established rapport with patient. Monitored vital signs every 4 hours. Provided comfort by assisting the patient into proper position. Taught the patient how to performed deep breathing exercises for relaxation. Performed bedside care. Provided safety by raising the side rails and lowering the height of bed. Administered analgesics as prescribed or as ordered by the doctor.

: After 1 hour of nursing interventions, the patients pain was reduced or controlled.
