BOOKWALTER RETRACTOR IN THE BEGINNING

The idea for the BOOKWALTER Retractor first occurred to Dr. Bookwalter in the middle of the night in 1964, when he fell asleep holding a retractor during an emergency operation. Upon awakening, he found that it had not mattered that he had fallen asleep and he began to think about the possibility of replacing hand-held retractors with a mechanical device.
Initially, he experimented with the Smith Retractor, a table-fixed retractor sold by CODMAN as early as 1954. It fastened to the table at two points and various retractor blades could be moved and tightened around the perimeter. While on active duty at Fort Bragg in 1970-1972, Dr. Bookwalter made extensive modifications to a Smith Retractor, and later showed these modifications to the people at Codman, with whom he was developing a pistol grip needle holder. Attention shifted to the retractor, and over the next 3 years from about 1975 to 1978, development progressed, ending with an over the drapes table post, an oval ring, retractor blades on ratchet bars with straight ratchet mechanisms. The ratchet idea had come from the Bailey Gibbon Rib Approximator. As soon as the product was on the market, it was obvious that improvements were needed. This led to the very early addition of the tilt ratchet mechanism, which allowed the retractor blade to toe in, much as the hand-held retractor was toed in. In 1985, under the direction of then product director, David Hable, the horizontal flex bar and the segmented rings were added, both of which have been continuously developed and improved to the present time.
In 1991, the tilt ratchet mechanism was modified so that the release was on the side rather than at the back. This was to facilitate using the ring for rectal, vaginal, and orthopedic surgery, where the ring was perpendicular to the floor. Moving the ratchet release to the side allowed both levers to be released with a single squeeze of the thumb. Initially, the levers overlapped. This allowed inadvertent release at certain angles and was superceded fairly quickly by our current design in which the levers are very close together but not overlapping.
In 2003, we introduced the most significant improvement to the retractor in the last 10 years. The ROTILT Ratchet Mechanism, rotates the tilt ratchet allowing the retractor blades to toe in, in planes other than the flat plane of the ring. It adds another degree of freedom, which is critically important in retracting deep tissues and permits precise blade placement while retaining the ability to quickly reposition the blade when necessary.
We move into the 21st century with continued commitment to provide the best table-fixed retractor in the world. Description With table post attached to the O.R. table at the level of the axilla, the horizontal bar end is positioned between the nipple and the sternum approximately 8 above the patient.
Once the horizontal bar is stabilized via the table post coupling the IMA cross bar is inserted and secured parallel with the patients chest.
The IMA retractors are inserted into the IMA ratchet mechanisms, and placed individually onto the crossbar. The crossbar safety clips are then locked.
The BOOKWALTER IMA Retractor Kit affords the surgeon specific and independent retraction for efficient dissection of the internal mammary artery.

Description The patient is in a modified prone position with the segmented spinal ring centered over the incision and attached to the horizontal flex bar.
As surgery proceeds through the skin and subcutaneous layers, shallow skin rakes retract these tissues.
As the para-spinal muscles are dissected, the shallow instruments are replaced with either deep rakes or Meyerding retractors.
Once exposure of the surgical area is achieved, multiple retractors can be positioned at any location on the segmented ring. This feature offers surgeons the flexibility to accommodate any size patient. This drawing depicts one method of achieving a wide lateral exposure over multiple spinal levels.

Description With the end of the O.R. table dropped, table post parallel to the floor, and the patient in the high-lithotomy position, the vaginal ring is positioned with the 20degree break at the level of the posterior vaginal wall.
The distal end of the horizontal flex bar is connected to the table post and tightened via the post coupling. Grasp the vaginal ring at the 5 oclock position with the horizontal flex bar, and secure.
With posterior (6 oclock), lateral (4 and 8 oclock) and Deaver (12 oclock) retractor blades positioned, optimal exposure is realized.

Codman offers 2 options for storing and sterilizing your BOOKWALTER Retractor systems and components. Whether your facility cleans and sterilizes instruments in storage/sterilization trays that are wrapped or rigid container systems are used Codman can assist you with your storage/sterilization tray/container needs. 1. Storage Sterilization: Codman has a variety of storage/sterilization cases designed to fit your specific BOOKWALTER Kits. The BOOKWALTER II cases provide an etched drawing of the exact layout of the components. This feature not only assists in the secure placement of each component but also insures that all components are returned to the kit after use. 2. Rigid Container System: Codman is pleased to represent the ACT2 Container system by Medin. This uniquely designed system offers many benefits: Manufactured from clear-coated aluminum they are lighter, more space efficient and stronger than other systems Smooth cam-lock latching mechanism insuring positive gasket seal Advanced filter system: rivet-less, puncture proof filter protection, an interchangeable filter plate provides easy to use filters and field replaceable gaskets. Tamperguard+ integrated tamper proof device and data card. Combines three functions into one, process verification, data entry area, and security Space saving design; only requires a 1-inch clearance between the inner basket and the container (competitive products often require 2 inches or more). Containers nest when stacked providing more space efficiencies The inner basket is laid out to specifically fit the BOOKWALTER Retractor System with an etched layout display to assist in replacing component after use.
