APPRENTICESHIP PROGRAM AGREEMENT AND LIABILITY WAIVER

KNOWN ALL MEN BY THESE PRESENTS: This APPRENTICESHIP PROGRAM AGREEMENT AND LIABILITY WAIVER, made and executed this 24th day of July at Bulacan, Philippines, by JETHRO A. DELA CRUZ, Filipino and a resident of #88B Maasim San Rafel Bulacan hereinafter referred to as the Apprentice.

WITNESSETH THAT, the B.S. Entrepreneurship Program of the MAPUA INSTITUTE OF TECHNOLOGY E.T. YUCHENGCO SCHOOL OF BUSINESS AND MANAGEMENT or MIT - ETYSBM, of which school the Apprentice is a student, has requested the Host Businesses to allow Apprentice to undergo Apprenticeship Program for a minimum of 8 hours per week for each Host Business as part of his/her academic requirement. THAT, the Host Businesses are willing to accommodate the request of MIT ETYSBM and to allow the Apprentice to undergo practical training, subject to the following terms and conditions: 1. During the period of his/her Apprenticeship program, the Host Businesses shall not be bound to pay the Apprentice any salary, allowance, compensation, stipend, or per diem. 2. The Apprentice shall abide by all rules and regulations observed by the Host Businesses employees during his/her Apprenticeship with the Host Businesses and shall exercise care and diligence in the task assigned to him/her. 3. The Host Businesses, Mapua Institute of Technology, its representatives and faculty shall not in any way be held liable for injury/ies and/or accident/s that the Apprentice may suffer in the course of his/her Apprenticeship. 4. Host Businesses, Mapua Institute of Technology, its representatives and faculty shall be held free and harmless from any liability for damage or injury to third parties which may be occasioned by intentional or negligent act of the Apprentice. 5. The Apprentice or, if a minor, his/her PARENT or GUARDIAN binds himself/herself to indemnify the Host Businesses for damages which may be caused on any property by the Apprentice intentional or negligent acts. 6. It is clearly understood that during the period of his/her Apprenticeship and presence in the Host Businesses, no Employer Employee relationship exists between the Host Business and the Apprentice. 7. This Apprenticeship Program Agreement and Liability Waiver will be in effect for the whole duration of the Apprenticeship Program.
