ABOUT THIS HANDBOOK


We have created this handbook to answer any questions you may have about the SLIM STUDIO and CSA dance. It contains important yearly information on our policies and procedures; outlining our philosophies and expectations and offers dancers and parents a clear understanding of their responsibility and commitments to the CSA dance program. Our policies and procedures are here to ensure that all students and their families are treated fairly and equally. It is important for you to read this entire handbook before your childs first dance class. Printed copies of this handbook can be ordered from our office for $5.00 each.

CSA HISTORY AND GENERAL INFORMATION


CSA HISTORY
CENTER STAGE AMERICA was established in the 2001. It was located in the small town of Ephraim, UT. In 2005, the dance studio moved to West Jordan, and this year after 5 more years of growth and success, the studio moved to its current location in Midvale. We attribute our success as a result of providing our students with a solid, technical dance foundation, running an organized business and an underlying belief in the philosophies of our program.
Center Stage America Dance is part of the SLIM STUDIO located in Midvale Utah. We are excited to announce the expansion of our studio and are looking forward to a great turnout! Our professional staff is looking forward to helping children and adults find a love of dance while also pushing them to become stronger more advanced dancers! We at CSA offer a wide variety of dance styles and classes for all ages, varying from age three to adult classes. As part of SALT LAKE INSTITUTE OF MOVEMENT we will be offering competition dance classes as well as more recreation dance classes for those who are not interested in the competitive side of things. We offer great classes at a competitive price and hope you will join our growing family!

Aspects of CSA DANCE


The Technical aspect; providing dancers a strong technical foundation in their choice of discipline. The Intensive aspect; designed for the students serious about dance, who learns to push themselves and wishes to broaden his/her dance technique in several disciplines. We love to work! The Competitive aspect (competition teams); Our Competitive Program has been expanded to multiple levels to provide competition opportunities to more students.

CSA prides itself on maintaining the professionalism and multiple aspect values we believe in. We hope that these will help encourage, motivate and teach your child to be the best dancer they can be.

The Summer Sessions: TBA will offer core technical training plus select specialty classes during weeks of summer. FACULTY Center Stage America is committed to providing quality instructors believing they are the key t o y o u r childs expe rience . Our staff consists of professional dance instructors with various years of technical training, BFA degrees in the field of dance, collegiate and professional performing experience as well as years of teaching experience. Under the direction of MaryAnn Champman, our faculty is committed to excellent technical training in classical ballet, jazz, modern, tap, hip-hop, ballroom, tumbling and more. FACILITY CSA understands the importance of providing a professional studio space for our dancers. All of our spacious studios have large mirrors, secure Barres and a professional floor. This combination helps protect our dancers from fatigue, pain and injury. LOCATION CSA DANCE is located in Midvale just south of 5400 south and State Street. Our studio location offers easy access for drop off and pick up as well as plenty of parking.

Because of your commitment to the process, you make an enormous difference. We encourage you to be a part of your childs dance education. Our program relies on a positive atmosphere for our faculty, dancers, and their parents; it promotes a positive learning experience for all involved.

Please Remember
Cooperation between all parents is expected. Showing respect for the other parents, along with the students and faculty, makes an important impression on the children. You are a role model for your child in how to interact with others in a professional setting. Your childs presence at all classes is important. The spirit of teamwork and the lesson of dedication are a big part of our studios educational process. Parents and teachers may look at a childs learning from different perspectives. However, they share a common goal: to assure that every child receives the best possible training, both physically and mentally. Mutual respect between our faculty and our dancers parents provides the children with the ultimate care and education. UNDERSTANDING EDUCATION DANCE

As a parent, the financial support of your student is important, but of equalperhaps even greatervalue is your emotional support.

Encourage your child to be the best that he or she can be without regard to what others may achieve. Dance is an individual art form; each child needs to achieve at a pace thats comfortable for him or her. No two students will progress at the same rate, even if they experience the exact same training. DOS DONTS AND DO encourage your children to give their all, to focus on themselves, and be joyful with their accomplishments. DONT compare your childs progress, choreography or placement with others in the program. Watch for this behavior in your children as well, and encourage them to focus on their own accomplishments - a negative focus or comparison distracts from the energy that could be focused on becoming a stronger dancer. In addition, class placement decisions are derived from many years of teaching experience. Often a child is placed in a particular group or class where he or she will feel confident, in order to promote the development of self-esteem.

Some dancers who are placed in a higher level become discouraged, only to lose their passion for dance. Others respond to the challenge of being in a class with dancers who are more proficient by pushing themselves to work harder. Placement is highly individual and the factors that go into the decision are complex. If you have questions or concerns about your childs placement or dance education, please do not hesitate to speak with the instructors of the class or Maryann herself.

DO look to others for inspiration. DONT speak negatively about your childs teachers, fellow dancers, or other parents in front of your childor other students this could result in problems far beyond your original concerns. Often children will react to their parents in a way that imitates the parents behavior with other adults or authority figures. If you do request a conference, please listen carefully to what your childs teachers have to say. They spend a significant amount of time with your child and can offer expertise in the field of dance education. CSA PARKING We ask that all parents and those who are dropping students off and or picking them up at S.L.I.M studio please do so in the East Parking lot, located behind the building. This lot can be accessed by turning at 6790 S (East Lester Ave). Parents please park in a designated spot and wait for your child. Please DO NOT pull up and wait in front of the studio door. The parking spots located at the front of studio are reserved for employees of the Salt Lake Institute of Movement and our businesses neighbors. Please be courteous and DO NOT park here for pick up and drop off. This entrance is NOT the public entrance or exit and we ask that you please be sure to respect the parking policies of the studio. Parents are responsible for their children before and after class times. Please do not drop your child off more than 10 minutes prior to class. Prompt pick up (within 5 minutes) is also expected.

CLASS OBSERVATION Parents are welcome to watch class from the waiting rooms or hallway through the observation windows. Only dancers who are enrolled in the class are allowed in the classroom during class time. P ar e nts a r e d i s c o u r a g e d fr o m b r i n g i n g o t h e r c h i l d r e n w ith them to observe classes or to wait in the lobby while their student is taking class as there is limited space and seating in the lobby. We understand at times this cannot be helped and we have provided a small waiting area room for those parents with children who are waiting for their student. THREE STRIKES DISCIPLINE PROGRAM If a child is found to be using disrespectful, dangerous or inappropriate behavior such as sitting or kneeling on lobby tables, jumping on and from chairs, running, playing relay or hiding games, rough housing, throwing food, clothing or other items, playing in the bathroom, distracting classes in session, defacing studio property, or other non-acceptable behavior, they will be given a written warning that will be sent home to you. If the child receives a 2 written warning, you will be requested to have a conference with their teacher or the studio director before they can return to the studio for classes. If the child receives a 3 written warning, they will be removed from all of their classes at Slim Studio/CSA DANCE. If the child is a sibling who is not attending classes, you will be asked to refrain from bringing the child to the studio while waiting for your dancer. Please know that this program is not instituted lightly, nor will it be used to judge or inhibit your childs appropriate behavior at the studio. It will, however, be a strictly enforced program, especially when children are endangering themselves or others. Please take this opportunity to speak candidly with your children about the appropriate behavior at the studio. Discuss in detail with them what is not appropriate, and the consequences of receiving a warning from the studio.

8 We want to provide the best possible facility, teachers, programs and environment for your childs dance education. For this reason we ask that you help us by following the rules and guidelines we have set forth in this handbook. If you have suggestions or concerns regarding any of the information in this handbook, please contact the office manager, or make an appointment to speak with me. DANCER ETIQUETTE AND GUIDELINES


The first step to becoming a successful dancer is making a solid commitment to your dance classes. Strong technique is a key ingredient and class is where you develop that technique. Come to each class prepared to learn and without personal distractions. Class time is your chance to focus on yourself, so try to leave your concerns or worries at the studio door. True progress is made when you look at each class as an opportunity to become better at what you love to do. Dance full out, stretch a little further, become more aware of your technique, and make the most of every class. Respect for the faculty and teachers you work with are essential. Listen to each correction given, whether its directed to you or another dancer. A correction is an honor; it shows you how much a teacher cares about your progress as a dancer. Always say thank you when a teacher offers you constructive criticism. Nothing can stop those students who apply themselves in every class and appreciate their teachers knowledge and experience.

DANCER ETIQUETTE
ATTENDANCE: Students are expected to be at all dance classes. It is important in the learning process that students are there so they do not fall behind or delay the progress of the class.

DRESS CODE: Wear proper class uniform, shoes and hairstyle. (See Dress Code section for more details).
ABSENCES: If a student is going to be absent or late, a phone call and or email should be made to notify the office or the students instructor. This will relieve any concerns that we might have of where your child may be. BE ON TIME FOR CLASS: Students should arrive five minutes early to class to get themselves fully situated before class begins. If a student arrives late to any class they should show courtesy and respect by asking the teacher permission to participate in class. RESTROOM BREAKS:

Use the restroom before class begins.

CLASS BEHAVIOR: The CSA Discipline Policy will be strictly enforced, and lack of adherence to its guidelines may result in removal from the program. Students are expected to manage their own behavior. CELLULAR PHONES:

Students cell phones should never be turned on while in the studio where a class is being taught or left on in the dressing room lockers. Constant ringing and voice mail alerts are disturbing to other patrons in the lobby area.

RESPECT THE TEACHER:

All students are expected to follow the directions of the teacher and to show utmost respect. Students should refrain from talking or visiting with classmates during class time. This is disrespectful to the teacher.

RESPECT THE STUDIO:

Students are also expected to contain their dance clothing and gear in a dance bag which should be stored in a cubby at all times. Items that are left strewn throughout the studio and floor will be deposited into the Lost and Found.

CLASS RULES o No gum, food and drinks are allowed in the dance studios. o o o o While taking class students should never stop dancing during an exercise unless they become ill or injured. Students should never sit down unless instructed to do so. Students should refrain from leaning, hanging, sitting or swinging on the ballet Barres. Students should always show respect for others in class by keeping their hands to themselves and staying in their own dancing space. Students should never leave a classroom to get a drink, use the restroom, or any other reason without asking the teacher permission to do so. Students should always end class by either applause or a personal thank you to their teacher.

DRESS CODE Grooming is essential to our professional image. Dancers who come to class looking and feeling like dancers behave accordingly. At CSA we follow a specific dress code to promote uniformity and a sense of professionalism as well as structure and discipline in the classroom. *Students will never be allowed to participate in a class in their street clothes.
ANY STUDENT ARRIVING FOR CLASS WITHOUT THE REQUIRED CLASS ATTIRE WILL BE REQUIRED TO C ALL THEIR PARENT TO BRING THE PROPER CLOTHING (within 15 minutes) IN ORD ER TO ATTEND CLA SS.

SHOES All new Pointe students (Pre-Pointe class) should NOT purchase shoes prior to their first class. Students who already have them may bring them. Pointe shoe fittings are highly individual, and will be discussed in detail in class. Your students instructors can help give you advice on options about where to buy etc.

Bear paws and foot undies may be worn by the students for lyrical and contemporary classes if teacher has okd them. Students in hip-hop classes may choose to wear sneakers to practice in, however we ask that these shoes be specifically for dance (please do not wear them anywhere but in the studio). Students who wish to participate in our ballroom classes may choose to purchase ballroom foot wear, but it is not required. (soft soled shoes are allowed) Students are allowed to use any tap or jazz shoes for class, however, all recital shoes will be ordered if necessary (if black and/or tan needed)

DANCE BAGS AND BELONGINGS Students are required to keep all of their belongings inside a dance bag. Please make sure that this dance bag is large enough to keep everything in and that your child only brings the things that are necessary to dance each week. A locker room is provided so that bags and belongings can be kept safely inside. All dance items (shoes, leotards and leggings etc) should be marked withyour childs first and last name before their first dance class. This will save us from headaches and tears LOST FOUND AND

There is a LOST and FOUND at the dance studio behind the main desk. Items that are left on the floor, in the studios, or loose in the lobby will be deposited into the Lost and Found. Items not recovered from the Lost and Found within 90 days are donated to Deseret Industries. If your child is missing any of their dance items, please check the LOST and FOUND before you purchase replacements.

13 CSA MERCHANDISE This year we will be offering the following dance clothing and merchandise: Warm-ups Wrap Skirts Duffle Bags & Totes Tights Hot shorts CSA Tees Tap Shoes Ballet Shoes Jazz Shoes & Pants

These items will be available to order at registration and multiple times throughout our year. All items require payment at time of order placement.

REGISTRATION
Registering for our classes indicates commitment to the entire dance year as outlined in this handbook. Registration for the Fall/Spring Session can be done at the studio or online at csadance.com

All registrations must be by t h e par e nt or guar d ian who is d e e m e d t h e r e s p o nsib l e p a rty f or t he a c c o u n t . The completed registration form, liability waiver, media release, and account contract must all be completed and signed by the accountholder in the studio. Only siblings living in the same household may combine on one account. Multiple families, even those living in the same household must set up separate accounts. The Liability Waiver Form must be signed waiving the right to any legal action for any injury sustained on CSA property resulting from normal class activity or any other activity conducted by students before, during or after class time. The following fees are due with all registration paperwork in order to enroll any student: registration fee $25 per student ($35 per household) First month free. No Refunds.

14 CLASS CHANGES st Class drops made after the 1 of month will not result in any tuition credit for that month. Class additions made during the month will result in a pro-rated tuition charge for that month. WITHDRAWAL POLICY Any student withdrawing from all CSA classes must submit a two-week written notice to the office before the end of the tuition month. Without written notification, tuition will continue to be billed, even if the student is not attending class. Withdrawals from single classes must be submitted in written form prior to the last business day of the month. No credits will be given for untimely notification. Withdrawal notices submitted after the 1 of the month will n ot result in a refund or credit for that month. Students wishing to withdraw by the end of the month must submit a th completed Withdrawal Notice by the 15 . Temporary withdrawals, due to injuries, serious illness, or unforeseen circumstances will be handled on an individual basis. CLASS MAKE-UP POLICY Make-up classes are available with the following stipulations: The make-up class will be scheduled for the same or lower skill level as the missed class. Students may not attend a HIGHER level class for make-up purposes. Make-up classes are not available for all classes, and are subject to change after recital preparation has begun. CSA reserve the right to remove the privilege of make-up classes, if that privilege is abused. CLASS POLICY CANCELLATION

CSA reserves the right to add or drop classes based upon enrollment, as

well as change the class instructor if necessary. In the event that a regular instructor must miss a class, a qualified teacher will be brought in as a replacement.

15 In the event of inclement weather, classes may be cancelled. CSA will cancel dance classes on days that schools are closed due to bad weather. If you have any questions about classes being cancelled, you should call the studio to hear a phone message if classes have been cancelled. Notifications will also be up on the website for easy access.

If classes are ever cancelled due to weather or for other reasons of unseen circumstances, a make-up class will be offered and accountholders will be notified by email of the upcoming make up schedule.

TUITION & FEES


ALL
ACCOUNTS MUST SUBMIT A VALID CREDIT CARD AT REGISTRATION. THIS CARD WILL BE USED TO CHARGE ALL TUITION AND DANCE-RELATED FEES DURING THE SESSION.

We at CSA have found it is easiest to have card information in our system so that payments can automatically be taken out at the correct time each month payment is due. This helps us to receive payments on time and takes away the stress of having to remember to get the payment in each month. Accountholders are responsible for providing up-to-date credit card information prior to all payment due dates (i.e. expiration dates, account changes, lost cards, etc). All card holders are responsible for contacting the CSA office before the last business day of the expiration month, and providing the new expiration date for their card. Grace periods will not be applied to credit card expiration defaults. Accountholders who do not provide up to date expiration dates prior to st the 1 of the month will be charged a non-refundable late fee. PAYMENTS RETURNED FOR NON-SUFFICIENT FUNDS WILL RESULT IN CREDIT CARD CHARGES OF THE BALANCE OWED PLUS A $25.00 NSF FEE. ACCOUNTS EXPERIENCING TWO NSF FEES WILL HAVE THEIR ACCOUNT STATUS MODIFIED TO CERTIFIED CHECK, MONEY ORDER, CASH OR CREDIT CARD PAYMENT ONLY. TUITION POLICIES All monthly tuition payments will be billed the 5th of each month. If the 5th occurs on a week-end or holiday, credit cards will be billed on the first business day following the weekend or holiday. (Credit cards will be automatically charged each month.)

Refunds for overpayment of tuition and/or fees will be given only for approved medical withdrawals. Invoices/statements will not be mailed monthly. Statement copies may be requested by email or in the office on an individual, as-needed basis. o There are no refunds on missed classes.

o Classes that are missed cannot be credited to the following month


or subtracted from the monthly tuition.

o There is no tuition reduction or refund for classes that fall on


scheduled holidays or breaks (see Studio Holidays). If your family experiences a financial emergency, please contact the office BEFORE your payment is due to make payment arrangements. Arrangements will be established on a case by case basis. Accounts with balances of 30 days or more will result in suspension from the program; a notice will be e-mailed to you temporarily withdrawing your child from classes until all fees are paid. No credits are given for classes missed due to lack on on-time payment. This policy is strongly enforced. LATE FEES

Accounts having overdue balances will be assessed a nonrefundable late fee of $25.00 or 5%, whichever is greater. Late fees are assessed each month on the remaining overdue balance until it is paid in full.

A ccounts having overdue balances or late payments for two

consecutive months will be moved to a pre-pay status and will be required to pay all fees 60 days in advanced for the remainder of the session.

PROGRAM FEES
ANNUAL REGISTRATION FEE $25.00 per student or $35.00 per family (three or more students) This fee initiates your account, reserves your childs space in class and is non-refundable. SPRING RECITAL PERFORMANCE FEE $50.00 per account. CSA performs their annual Spring Recital each June. The recital w i l l b e performed at Murray High School, 5440 S State Street. A non-refundable performance fee of $50 is assessed to each account and is due and ST payable by October 1 with October tuition. This fee covers Spring Recital expenses such as administrative fees, insurance, building rental, ushers, janitorial fees, technician fees, programs, etc. This fee entitles each family to FOUR complimentary reserved tickets to your choice of one of three performances, and a rehearsal CD. Additional tickets will be available for purchase at the front desk. The price of these extra tickets will be posted at the studio. SPRING RECITAL COSTUME FEES Costume Fee; TBA These fees will be determined based on the costume that your childs instructor chooses. These costumes will be ordered prior to the recital date and will be worn by your dancer for dress rehearsals and the final performance.

MONTHLY TUITION Costs vary per student and program CSAs Fall/Spring: Monthly tuition remains the same whether there are 3, 4 or 5 weeks in the month. The longer months, extra rehearsals and recital tech rehearsal in the spring months compensate for the shorter months, observed holidays and breaks). All observed holidays and breaks have been incorporated into the monthly tuition rates. (Please refer to the holiday list on the inside cover of this handbook).

COMPETITION The competitive program provides an avenue for dancers to learn teamwork and good sportsmanship, benefit from healthy competition, and develop stronger performing skills. Competition teams have the opportunity to compete/perform during the spring months at competitions, conventions and fundraisers. Auditions for Comp teams are held each year. Dancers who are selected are required to sign up for a designated schedule of fall classes. Students who are interested in auditioning for this program should consult with the Studio Director or other instructors. Additional expenses for Company members m a y include, but are not limited to: Choreography, costumes, accessories, make-up kit, jewelry, convention and competition fees, warm-ups and duffle bag.

FAMILY DISCOUNT Families (on a single account) with two or more students enrolled in CSA classes will receive a family discount.

The discount is calculated by multiplying $5 times the number of additional students. (The first student is not discounted) This discount is then applied each month to the families tuition as long as the students are actively enrolled in class and tuition is paid on time. Family discounts may be revoked if tuition and all fees are not paid on time.

SPRING Information
Pictures: April 20 th @ Jaylynn Studio Recital Technical Rehearsal: Saturday May 25 th Recital: Tuesday May 28 th Please plan to set aside the entire day for these events. These events are mandatory for all performance class students. Specific time schedules will be available after the first of the year. Please Note: Additional rehearsals are always scheduled during the months of May and June and due to class schedules, are frequently on Saturdays. Please be aware of this possibility. No additional tuition is charged for these rehearsals. Successful recital preparation requires consistent attendance by all students. Students can, at the teachers discretion, be removed from any or all recital pieces if repeatedly absent or not prepared due to lack of rehearsal.

COSTUME DISTRIBUTION DATES Recital costumes are distributed only to parents by the Retail Coordinator during designated hours. These dates and times are listed below:

PLEASE PLAN TO PICK UP ALL OF YOUR STUDENTS COSTUMES DURING ONE OF THESE TWO SCHEDULED TIMES. Accounts with unclaimed costumes will be billed a $10.00 pick up fee to schedule an appointment with the Retail Coordinator to retrieve their costumes. The office staff will be available to distribute costumes during office hours. All accounts must be paid in full in order for students to take home their spring concert costume(s) and participate in the annual spring concert. This includes all monthly tuition, late fees (if issued), concert fees, costume fees and uniform fees. There will be no exceptions made to this policy. Costume deposits are due November 5th, 2012. The deadline for withdrawal from CSA classes in preparation for the 2012 Spring Recital is Friday, December 17, 2012. Costume deposits will not be refunded after Monday 10th.

COMMUNICATION
Communication between the studio and parents is essential. The foremost communication tool for the studio to communicate with parents is through email. Information is also posted regularly on the website, and through quarterly newsletters posted on this site and emailed to all accounts. Additionally, CSA posts calendars and notifications on the bulletin boards, sends notes home with students, and mails letters to parents when needed. Parents are asked at registration to provide a valid email address so that CSA may communicate time sensitive notices as well as Parent Handbooks, Newsletters and other important tools of communication. Parents are responsible for keeping apprised of studio schedule changes, events and notices through the timely reading of emails, quarterly newsletters, and the UDA News bulletin board. NEWSLETTERS Newsletter will be emailed to all active accounts on a regular basis. Please take the time to carefully read all information that is in the newsletter as most of the important information, deadlines and event dates you will need is contained there. This is our key component of communicating with our parents. STUDENT RECOGNITION Center Stage America is very proud of all of our students accomplishments. Throughout the dance year if your child receives recognition in dance or the visual arts, please inform us so that we can announce it in our studio newsletter.
