INTRODUCTION Community Health Nursing is the practice of promoting health and preventing illness at the community level using the knowledge from nursing, social, and health. It is a continuous process which aims at rendering effective health care to the people in the community. It includes assessment, diagnosis, planning, implementation and evaluation. The family is considered the basic unit of the society. It is in the home that the individual members of the family grow and develop not only physically but also psychologically, socially and spiritually. The environment is also a major factor in the total development of a person. His interaction with his environment affects his decisions, values and attitudes. Thus, the family, home and environment where a person lives determines how hell grow and develop totally as a person. It should be conducive to health maintenance and personal development. As health advocates, the NCM501106 nursing students of Liceo de Cagayan University has the duty to promote the health of the community starting from the family and its individual members. This is done by rendering effective health care services to promote health and prevent the occurrence of illness. This is done to improve their health status and to identify and find ways to resolve their potential and existing health problems. We chose the family of Mrs. Reyes for our family care study since they fit the criteria for selecting a good family care study. They have one child and one of the survivors of the bagyong Sendong and stayed at the West City Central School as evacuees. We were able to identified health problems that need attention and its our responsibility to address these problems identified. This will help in promoting the health of the Reyes Family as well as contribute to the improvement on the quality of health in the entire community.

II. A.OBJECTIVE of the STUDY This study aims to use the nursing process beginning with the assessment to identify the health problems of the individual members as well as the whole family so that a proper diagnosis could be formulated. We will be able to plan the appropriate care for the family and thus implement the said plan to help alleviate if not solve the problems of the family. An evaluation of the effectiveness of the care rendered will help in the changes to be done to ensure the affectivity and continuity of care for the family. This process will help to improve the health condition of the family as well as their quality of life through the mutual cooperation of the whole family and students. B. SCOPE and LIMITATIONS of the STUDY The study was conducted at West City Central School, Carmen, Cagayan de Oro City in which we were given only two days of visitation in caring the family. The study focuses on their environment and a child below 5 yrs old, specifically the disease they are feeling these days. The data gathered was based on the observation of the health status and interviewing the mother. The care study includes the identification of the family members, description of their living environment, health and financial status of the family members, including health insurance and patterns of health care utilization, health beliefs, educational level and aspiration of the family.

C. SPOT MAP From the campus of Liceo de Cagayan University, the students rides any jeepney (R1, Bonbon, Kauswagan) that goes to KSY, Carmen with a fare of Php6.00 for students and Php 7.00 for regular fare. When you reached KSY, you will then ride a tricycle and ask if they will go to West City Central School, if so, the fare will be again Php 6.00 for students and Php7.00 for regular fare. Once youre in the school, right side building where the evacuees stayed you will find the family of Mrs. Reyes in room No. 057.
