Tatay Mimong is a 79 years old senior citizen who has 3 children and has 4 grandchildren. He was widowed 3 years ago, his wife died due to complications of diabetes mellitus. Tatay Mimong is currently living with his eldest son. He shared that he has a family history of hypertension, asthma and diabetes mellitus. One of his siblings died due to tuberculosis. Currently, Tatay Mimong has asthma, DM and pneumonia. As he verbalized he has asthma when he was 18 years old, DM was diagnosed when he was 65 when he have his check-up and a year ago of pneumonia. He admits that he is not compliance to drug and does consult doctor if in severe condition.

1.) What are the important factors affecting health status? Tatay Mimong said that due to his lifestyle that affects his health status. He started drinking alcoholic beverages and smokes at the age of 15 almost every day. He can consume half per day. And due to a unhealthy diet he said that he eats foods rich in fats and seldom eats vegetables. He also said that he is not athletic a does not exercise regularly.

2.) Which of the identified health concerns are due to aging? Tatay Mimongs health concern due to aging is the physical changes. He feels weaker not compare when he was young where he can carry or lift heavy baggage but now due to old age he has weaker muscles and bones. His DM because he doesnt have maintenance for his DM. Another one is his asthma because of the decrease in immune system he is now dependent in his inhaler for asthma. All of his siblings have asthma since his mother has also asthma and died due to a lung problem.

3.) Which of the identified health concerns are due to presence illness? His current concern is about his asthma, DM and diagnosed pneumonia he complies with meds but didnt have a follow up check-up. Though he said now days he seldom eats meat and food rich in carbohydrates and sugar. But he is still worried because he knows that there are many complications of DM that may lead to death if not manage and prevented soon.

4.) What type of living arrangements should be provided for him? A type of living arrangement should be provided for him is a type of living where he is comfortable with a good environment. According to him he is satisfied and contented in where he stays now with his son and grandchildren. Every weekend his other children also visit him and all of them shares for the need of their father. He describes the environment where he stays as simple yet comfortable. He can have enough rest and can eat 3 times a day not compare to their own house where he lives alone after his wife died thats why his children decided to let their father stay in their home for a more comfortable environment. 5.)
