1.0 Project Background

Andhra Pradesh (AP) state has one of the longest coastal belt in India. Out of 67,362 Km of road network in the state, the nine coastal districts cover 26,000 km of road length. Every year during the period of April-May and September-December, cyclonic situations set in to the Bay of Bengal. With the recent changes in climatic conditions, the catastrophic events are increasing resulting in very devastating cyclones like Laila, Jal etc where the loss is enormous. In the 97 years ranging from 1892 to 1989, there were as many as 67 cyclones within the State. It is essential to provide safe residential accommodation to the vulnerable population during any eventuality or disaster due to natural calamities such as cyclones and associated tidal waves and storm surges. The project is formulated based on experiences of Government of Andhra Pradesh in mitigating such disasters during the past and also keeping in view the existing facilities available in the project area in Andhra Pradesh. The Government of India has applied for a credit of US$ 255 Million equivalent from the International Development Association (IDA) toward the cost of National Cyclone Risk Mitigation Project (NCRMP), and it intends to apply the proceeds to payments for goods, works, related services and consulting services under this Project. National Cyclone Risk Mitigation Project (NCRMP) Project Implementation Unit (PIU), Andhra Pradesh, headquartered at Hyderabad has invited the consultants to submit proposals for providing the consultancy services for Third Party Quality Audit for 9 coastal districts namely Srikakulam, Vijayanagarm, Visakhapatnam, East Godavari, West Godavari, Krishna, Guntur, Prakasam and Nellore. The project Construction of Multipurpose Cyclone Shelters, Roads to Cyclone Shelters, Roads to habitations and Roads & Bridges under NCRMP is funded by International Development Association and received through Government of India. This consultancy is envisaged to audit the quality of works being implemented by various line departments working under PIU. The number of works under progress / for which
agreements are concluded in each district is shown in the table below.

The district wise progress of works is shown below. The works in the West Godavari are not started and the works in Krishna District has just started. At many places the first milestone is completed but works are not started. The line departments are to analyze the reasons for the slow progress and if necessary penalty for the slow progress as per the agreement conditions should be imposed.

6.0 District wise status of tests conducted by TPQA: Apart from monitoring the tests being conducted by the implementation team (contractors & Line department) at various stages, the Third party Quality Audit team also conducted tests to confirm its compliance with the standards. These tests are carried out in the site itself and in some cases samples are collected and testing done in outside laboratories. The details of tests conducted in the field till end of September 2011 are presented.

3 The width of shoulder formation is verified from 0/0 to 2/250 Km., Shoulders portion in some areas have slipped and the same may be rectified. Formation width is less than 7.50 mts from Km: 1/600 to 1/900, & from Km:2/350 to 2/530. From Km:1/600 to 1/900 the road level is lower than the surrounding fields level, and in this stretch sand may occur in monsoon period, this aspect needs examination. The tank bund portion from Km 2.350 to 2.530 , the height difference between road top level and field level is more than 3.0mts. But no slope protection work is provided. Safety measures like providing caution boards and providing helmets is to be done. White wash shall be done to parapet walls of the existing culverts for the clear visibility of vechicular traffic. Some of the KM & HM Stones are missing in this working road stretch. The field officers are requested to erect them immediately. Old narrow pipe culvert between Ch:1/100 to 1/200 were not replaced and it may be prone to accidents. With this speed it is very difficult to complete the work within the Agreement period. The field officers are requested to get the samples of steel tested from every consignment of steel , before it is used. The steel should be protected from rusting till the concrete is placed by applying cement coating properly as per clause 1603 of MORT&H standard specification. Steel and cement stock register should be maintained with proper incoming and usage recording should be maintained. The existing bridge being used for directing traffic it should be properly barricaded on the either side as most of railing got collapsed. Reflective caution boards should be erected at entry and exit of the bridge.
Safety measures for workers such as Helmets, Gum shoes, jackets etc., are to be provided by the contractor. Caution boards, sign boards are to be kept. The field officers have not furnished the copies of the Bills of Cement and Steel utilized on the bridge. The same shall be furnished to the TPQA staff during the inspections for verification.

Measurements of piers, bed blocks and abutments are verified and found to be fairly tallying. At some locations, honey combing is observed at some places. Finishing shall be carried out as per clause 1714 of MORT&H standard specifications for Road & Bridge works. Reinforcement grills for one deck slab are verified with reference to the drawing and found to be fairly tallying.

Progress of work: 2 abutments and 7 piers completed upto bed block level. Bed block over one pier is completed. Site order book is not being maintained by the field officers. The field officers are instructed to maintain invariably the site order book and get the instructions from the inspecting officers. The dimensions of bed block laid over P5 are measured and found taillling with drawing, but the bed block offset all around the pier is not seen as it is flushed with the pier. The pier is bluged at the bottom of bed block, because of which the offset is not seen.(Photograph attached.) Proper Shuttering shall be used and finishing shall be carried out as per clause 1700 of MORT&H Standard specification for Road and Bridge works. The field officers are requested to get the samples of steel tested from every consignment of steel , before it is used. The steel should be protected from rusting till the concrete is placed by applying cement coating properly. The overhead electrical lines passing across the structure should be shifted as it is posing danger to proceed further above the piers. Safety measures for workers such as Helmets, Gum shoes, jackets etc., are to be provided by the contractor. Caution boards, sign boards are to be kept. The field officers have not furnished the copies of the Bills of Cement and Steel utilized on the bridge. The same shall be furnished to the TPQA staff during the inspections for verification. As daily backwaters flow in the Perali drain, curing the structure with the this water should be avoided. Samples of cement and steel are collected by TPQA staff and sent for confirmatory testing. The Field officers are requested to provide village name boards, sign boards &caution boards. The joints in the CC pavement are not constructed as per Agrrement specifiction. All the joints are formed with mastic pads. It seems that nieither the contractor nor the line departmental offficers went hrough the agreement specification. It may not cause structural problem but the appearance is not very good.
