Stained Amniotic fluid Investigator: Jhanice T. Sangda-an, M.D./Department of Obstetrics and Gynecology Purpose and Conduct of the Study Good day! I am Dr. Sangda-an and I would like to invite you to participate in a research study. The study aims to determine the obstetric and perinatal outcome of term pregnancies with meconium-stained amniotic fluid noted in the deceleration phase of labor. The parturient in the deceleration phase of labor noted to have meconium stained amniotic fluid will be asked to participate in the study. Upon entry of the study, you will be interviewed and the following steps will be undertaken: 1. You will be asked to lie flat or on supine position on the bed. 2. The parturient will be hooked to an external fetal monitor or a cardiotocogram (CTG) machine by identifying first the fetal heart tone using a stethoscope. 3. After proper placement of the Doppler and Tocodynamometer part of the CTG machine on the abdomen, the procedure will run for about 20 to 40 minutes. 4. Patient will be asked to lie on her side, change position or to be administered with oxygen depending on the result of the CTG strip. 5. The CTG strip will then be interpreted so that appropriate action will be done. 6. The participant will then be monitored as to the type of delivery, either in the form of vaginal or abdominal delivery. If the patient delivered abdominally, the indication for such procedure will be noted. 7. The baby will then be monitored and followed as to the APGAR score upon delivery, perinatal morbidity or mortality up to the 28th day of life. Possible benefits for the participants Your participation in this study is of your own free will and may not benefit directly from it. However, the payment for the CTG is free of charge and we can

expedite the release of result. Furthermore, this study may help the birth attendant or obstetrician in decision making regarding the management of parturient women with meconium stained amniotic fluid in the deceleration phase of labor. This may benefit future patients with similar condition. Risks and inconveniences External fetal monitoring using a cardiotocogram is a safe procedure, although you may experience discomfort during the procedure, which is part and parcel of the procedure and we will take this into consideration and with extreme caution. Provision for injury or related illness If you experience any medical problems related to the study, the investigator will provide the necessary care and treatment. Voluntary Participation As mentioned earlier, your participation in this study is of your own choice and of your own free will. There will be no sanction on your part if you decide not to be included in this study. You can withdraw from the study anytime without sanctions. The investigator can withdraw you from the study as he sees fit for safety reason. Confidentiality All data collected during the study will be kept confidential. Your identity will be known only to the investigator who conducted the test. The corresponding Ethics Committee involved would have access to your medical records to check if all procedures are being followed. Your name will never be used in any publication or presentation of this study. CONSENT FORM I have read the information in this form as explained to me by Dr. Sangda-an and I fully understand everything that would be done during the course of this study. I

have asked questions about the procedure and any questions that I asked have been answered in full. I have been informed that my participation is voluntary and I can withdraw my consent anytime for whatever reasons. After all the details of the study have been explained to me by the investigator, I was asked by a non-member of the trial team if I was ready to sign the consent and participate in the study. Ako po si Dr. Sangda-an at inaanyaan ko po kayong lumahok sa aking pag-aaral. Ang pag-aaral na ito ay naglalayong alamin ang kakalabasan ng pagbubuntis ng isang babae na nag-lalabor, kung saan ang sanggol ay nakatae sa loob ng sinapupunan. Gagawin lamang natin ang prosesong ito bilang pag-aaral. Sa pagsali sa pag-aaral na ito, ikaw ay papanayamin at mga susunod na mga pamamaraan ay gagawin sa iyo: 1. 2. Ikaw ay hihiga sa kamang paanakan na kung saan ikaw ay pakakayangin. Lilinisin ang iyong pwerta gamit ang povidone iodine. 3. Ikaw ay ikakabit sa cardiotocogram para malaman ang pattern ng tibok ng puso ng sanggol at intensidad o regularidad ng pagtigas ng matres mo. 9. Humigit kumulang sa bente hanggang kwarenta minuto ang tagal ng pagkakakabit ng cardiotocogram sa iyo. 10. Aalisin ang cardiotocogram na nakakabit sa iyo at babasahin ang resulta ng pagsusulit.

11. Pagkatapos ng lahat ng ito, ikaw ay dadalhin sa delivery room para eksamining maigi ang luwang ng matres mo kung malapit ka nang manganak o hindi.
