Participants or providers complete a health profile, which places users in appropriate messaging protocols and personalizes their experience based on their physicians recommendations. Personalized messages and medication reminders encourage adherence and educate the user about their condition. Participants record progress against weight and exercise goals, blood glucose readings and blood pressure results. User inputted data builds into a comprehensive log. Personalized feedback and encouragement is given to support personal goals.
Participants portal houses entered data and displays it on an intuitive set of dashboards designed to show progress and highlight areas requiring attention. Data from the portal can be shared with the participant's care team (printed reports or emailed from the system).
PROVIDER INTEGRATION
Published Application Programming Interfaces (API's) can integrate the service with case management systems or electronic health records so the participant's care team can access their data and review progress.
