Chiari Type I develop as a result of: 1. Medulla and upper spinal cord compression results to myelopathy and lower cranial nerve and nuclear dysfunction 2. The cerebellum is compressed results to ataxia, dysmetria, nystagmus and dysequilibrium 3. CSF flow is disrupted to the foramen magnum results to the most common symptom PAIN (headache and neck pain). Aside from pain, it also would lead to the formation of syringomyelia and central cord symptoms such as hand weakness and dissociated sensory loss. Headache and neck pain in Chiari type I malformation are aggravated by cough and valsalva maneuver. Chiari Type II In Chiari Type II malformations compressive mechanisms, medulla, upper spinal cord and cerbellar compression, also plays a vital role in the occurrence of the disease. However, additional mechanisms are present that contributes to the occurrence of the malformation such as the stretching of the abnormally oriented cranial nerves. Presence of hydrocephalus, as a result of the myelomenigocele further aggravates the downward displacement of the brainstem and the stretching of the cranial nerves. Type II has poorer prognosis than that of type I after surgery because of the irreversible ischemia of the brainstem which is constantly under tension.
