of De.ce111ber , 201111fyear), at 811115 SW 6th St Noaoth Laude a-dale Ploaoida (ctlmeiOcatlon) the above named
defendant committed the above offenses charged and the facts showing probable cause to believe the same are asfollows:
On 12-21-1118 responded to SW 8111th Tea-a-ace SW 6th St. to conduct a DUI . Upon
arrival I 111et with Deputy Payne CCNI 148?8. Deputy Payne was dispatched to the area in
reference to a Red Pard SUU that was driving reckless, running traffic lights and traveling at
a high rate af speed. As Deputy Payne was at Ki111berly Blvd SW 81st Ave looking out for a red
Pord SUU she observed a vehicle approaching the intersection headed northbound on SW Btst Aue
with its lights off. Deputy Payne aduised she notice the vehicle was a red Pard SUU traveling
at a high rate of speed and 111atched the description of the vehicle that was dispatched as
driuing reckless. Deputy Payne got behind the SUU and activated her ouerhead lights and
followed the vehicle for approximately three to foUl" blocks and the vehicle 111ade no
to stop. Deputy Payne advised as the vehicle traveled north on SW Btst Ave, the vehicle
swerved in and out of lanes seueral ti111es. The vehicle then turned into a residential
neighborhood and pulled into the driveway of 811115 SW 6th St and the Driver, Defendant, Uincent

defendant committed the above offenses charged and the facts shoWing probable cause .to believe lhe same are as follows:
Ricbnond stepped out of the vehicle and Deputy Payne nade contact with the Defendant. Deputy
Payne aduised she could snell an odor of alcohol on the breath of the Defendant when she Made
contact with bin.
When I nade contact I noticed that the Defendants eyes were blood shot- speacb was slurred and
a uery strong odor of alcohol was coning fron the breath of the Defendant. I asked the
Defendant if be bad been drinking. The Defendant told ne that he bad a lot of alcohol to drink
and he could not renenber bow nucb. The Defendant advised ne that he did not had no nedical
condition and is in good health.
I asked the Defendant to perforn a field Sobriety Bxcercise. The following occured during the
excerc ise:
HGN: The Defendant eyes tracked equally. The Defendant both eyes had a distinct nystagnus at
naxinun deviation. Both eyes had an on set of nystagnus prior to 45 degrees. thye Defendant had
to be told to track the stinulus with his eyes and not his head several tines.
One Leg Stand: The Defendant placed his foot on the ground before 18 seconds and could not do
the excercise.
Walk and Turn: The Defendant nearly fall three tines in the start position.
The Defendant was place under arrest and taken to the BAT to submit to a breath test.
