GORDONS LEVEL OF FUNCTIONONG HEALTH PERCEPTION- HEALTH MANAGEMENT PATTERN He usually self-medicates whenever he had problems with health

particularly cough, colds and fever. He rarely visits the doctor to have a checkup and seek for medical assistance. He takes over the counter drugs such as biogesic or neozep for these conditions. He cannot comply with the drugs prescribed and all the doctors orders. When sick, patient waits for illness to heal, and if not bearable client goes to the hospital for a check up. NUTRITIONAL-METABOLIC PATTERN His usual diet is consisting of rice, fish, vegetables, and sometimes meat. He eats three times a day (breakfast, lunch, dinner). Meal pattern, breakfast: 1 cup of bread, and coffee with a match of cigarettes, lunch: 1-2 cups of rice, viand and sometimes with vegetable; dinner: usual cups of rice and viand. He usually takes a snack in the afternoon consists of coffee with a match of cigarettes or anytime of the day, takes 3-5 bottles of 12 oz. softdrinks . The client takes a bath daily. He does not use any sunscreen or lotion. He started drinking alcoholic beverages, specifically tuba at the age of 17 years old. He can consume 1/2-1 gallon of tuba every session or 4-5 alcoholic bottles big as a pilsen, and consumes 3-5 packs of cigarettes per day. He drinks coffee twice daily. During hospitalization, client recommended to cease smoking and drinking alcoholic beverages and any kind of sweet beverages. ELIMINATION PATTERN Urinary pattern of the patient is regular with no pain or burning sensation. Client defecates once a day usually brown and formed. No bowel surgery experienced. Doesnt use laxatives to aid in bowel movement, and urinates several times per day. No history of bladder surgery. During hospitalization, client doesnt have any complain pertains to any changes in urination or defecation. ACTIVITY-EXERCISE PATTERN The client does not follow a regular exercise regimen. He considers his work as an exercise, and do lot of activities as he can. During the course of illness, client stop in taking work and rest for couple of weeks, but prolonged pain caused him to seek consultation. SEXUAL-REPRODUCTIVE PATTERN

Single and still leave with his parents together with his other sibling. He has neither history of sexually transmitted disease nor any disease affecting his genitals. SLEEP-REST PATTERN Before admission, the client usually sleep pattern is disturbed with no continuous long duration of sleep with just approximately 30 minutes-2 hours due to the pain felt at the big right toe. He does not have any bedtime rituals. During his admission, he had continuous difficulty falling asleep because of his present condition and tends not to lie down because of the pain. SENSORY-PERCEPTUAL AND COGNITIVE PATTERN The client doesnt wear graded eyeglasses and does not have difficulty in hearing. He can take the food well. He is oriented to time and place and knows the people around him. He cooperates with the interviewers. His recent and remote memory is intact. ROLE-RELATIONSHIP PATTERN The patient doesnt have an own family yet, and still leave with his parents together with his other sibling. The other 6 siblings have an own family and leave with their own houses but still with the same place. They respect each other and follow the rules the patient has made. They still had an open and continuous communication with each other. During hospitalization, they seek and helped by other family member for the expenses for hospitalization. SELF-PERCEPTION-SELF-CONCEPT Patient manages to still practice used lifestyle despite of the disease he had. Recently they believed that admission will be helpful to assist him in his needs, to alleviate the pain he felt and for what appropriate recommendations to be. COPING-STRESS TOLERANCE PATTERN The clients major stressor is being admitted in the hospital due to its financial concern. He copes with this by spiritual means and with the help of his family but he knows it is necessary for him to undergo medical treatment.

VALUE-BELIEF PATTERN The patient does not usually go to church every Sunday nor is he active in church. Whenever he has a free time, it is the only time he goes to church to attend the mass
