Vendor Application 2013 Thank you for your interest in The Beverly Farmers Market!

The Beverly Farmers Market Board of Advisors welcomes returning and new applicants to the 2013 season. Opening day is on Monday, June 10, 2013! Please, use this page as a guide to the application process. Feel free to contact Estelle Rand at 978-473-9891 or estelle.masse@gmail.com if you have any questions during this process.

Instructions
Please take some time to review the following important BFM 2013 forms: Details Guidelines Vendor Application Vendor Agreement Please submit the following for a complete 2013 BFM New Vendor Application: Vendor Application 2013 (two pages) Signed Vendor Agreement Product lists Copy of pertinent licenses and permits for food production if applicable Upon Acceptance: You will receive seasonal permit form from Beverly Board of Health (if applicable) Send Payment: Check payable to The Beverly Farmers Market, due upon acceptance Return all application materials to: The Beverly Farmers Market 14 Central Street, #2 Beverly, MA 01915. Review of applications will begin on March 1, 2013. You will be notified when we receive your application and again in reference to your acceptance. We look forward to the 2013 BFM season with you! The BFM recommends participation in the FMNP; for more information on eligibility please contact Lisa Damon at 617-626-1731 or Lisa.Damon@state.ma.us. Food Vendors The Beverly Board of Health requires that you provide the farmers market with copies of pertinent licenses and kitchen permits. Please, send a copy of your permits and licenses with your completed application. The BFM will send a copy to the BOH. In addition the BOH requires that all food vendors hold a permit from the City of Beverly. A seasonal farmers market permit is available for out-of-town food vendors for $25. Upon acceptance as a BFM vendor you will receive the necessary BOH permit application with instructions in the mail. Optional Environmental/Community Statement/Tag line:
