Ms.

C is a 40 year old female who came in with chief complaint of redness on her left arm

Ms C is a 40 year old woman with a history of allergies to penicillin, cephalexin, codeine, and triazedone. She had blood work Admission on her arm 1 week ago. On Monday after presenting erythematous redness, she was given doxycycline, ibuprofen, and warm compresses. The treatment was ineffective, and she came down with cold sweats and vomiting. She was admitted to the ER for cellulitis and abcesses, which was drained and treated with vancomycin. Thanks Put later

She has a history of fibromyalgia, asthma treated, hypertention, heartburn, chronic bronchitis, and a herniated disk. Her medications include Cymbalta, ibuprofen, verapamil, omeprazole, tramadol, albuterol, clondipine, and sometrol. *allergies* Her past medical history includes appendix removal at age 11, gall bladder removal, and a hernia surgery at age 14. Her father had asthma Her mother had heart disease, hypertention, and bladder cancer Her aunt had MI at age 40. She has 2 daughters, 1 of which has rheumatoid arthritis. She smokes pack a day, does not drink, and reports a supportive social environment. She has an upbeat attitude. She excercises occasionally with kids and reports eating moderately well. She is disabled on social security. Statrement

Review of system concluded no chest pain, palpitations, urinary problems, or stoumache pain. Aortic palpitation reveals normal sounds. She reports bedsoreness from hospital stay. Her heart sounds generally well with normal S1 and S2. and her pulse is normal. Her blood pressure is high at 150/80 Lung sounds and physical exam was inconclusive. She is neurological awake and aware with normal strength. Her left arm is warm with a hardness in the center of the ellow joint and soft raised tissue radiating dsf Ipression

TP 40yo F Arm redness over the weekend on L arm (she had had blood work done on that arm 1-1 1/2 wk ago), very sore and warm. Went to doc on Monday was given doxycyclin and tried warm compresses and ibuprofen 800. None of it worked, then she felt feverish, cold sweats, vomiting but no streaking up arm. Went to ER Review of system Monday, admitted for cellulitis, had abscess. Was drained and she was given vancomycin PMH: herniated disc, fibromyalgia, ashtma, HTN, chronic heart burn, chronic bronchitis, has an IUD meds: cymbalta, ibuprofen 800, verapamil, omeprazole, sometimes vit D3, tramadol, albuterol, clonipin, solmeerol psh: appendix out age 11, gall bladder removed, hernia surg in '94 alelrgies: penicillin, cephalexin, codeine, triazedone (gi upset) FH: dad had asthma. Mom heart disease, HTN, bladder cancer; maternal gma HTN, diabetes and heart disease. Aunt had MI at 40. 2 daughters 1 has RA SH: smokes 1/2pack day, no drinking, lives with partner of 12 y (male), unmairried, has done weight watchers. Disables on SS, exercises w/ kids, lots of supports, 1 daughter college, 7yo daughter, 9yo son ROS: no chest pain, no palpitations, no urinary problems, no stomach pain, feeling generally sore PE: erythematous, RR 22, HR 60, well appearing. Youre gonna have to fill in the rest of the physical exam b/c I don't remember al lthat and didn't write it down.
