Summary: The applicant, after printing two copies of the copyright application
form, fills-up the copies of the form and submits it to the Notary Public. The
Notary Public then notarizes the document and after the applicant pays for the
notarization fee, the document is then returned to the applicant.

Post-Conditions:
Summary: The applicant, after having notarized two copies of the copyright
application form, purchases two documentary stamps from the COOP Staff. After
purchasing the documentary stamps, the applicant attaches the stamps to the
forms, and submits it together with two copies of the applicant’s work to the
Registrar. The registrar then checks the applicant’s work and gives the applicant
a payment slip. The applicant brings the payment slip to the Cashier and pays
the required amount. The Cashier then gives the applicant a customer copy of
the official receipt of his payment, which he then submits to the Registrar. The
Registrar then confirms the payment and informs the applicant as to when his
work can be claimed.
