APPLICATION PROCEDURE 1. Fill out application form and attach 2X2 and 1X1 recent colored I.D. pictures. Application Form is available at the Guidance & Counseling Office of the Institute of Medicine, from 7:30am 5:00pm Mondays thru Fridays. Application form can also be downloaded from our website, www.feunrmf.ph. 2. Proceed to the Office of the Undergraduate Degree Program you are applying for and submit complete requirements for Admission together with the accomplished Application Form with pictures, from Mondays thru Fridays 8AM to 4PM.

Nursing Applicants should personally submit the requirements for 3 Pay the Application Fee of P600.00 (non-refundable & nontransferable) to any RCBC Commercial Bank nationwide, MondayFriday, 9:00-3:00pm, use the Bills Collection Slip or to FEU-NRMF Institute of Medicine Building Accounting Office LG-7, Monday-Friday, 8:00am-4:00pm 4. Present the Official Receipt/Bills Collection Slip of RCBC to the Office of the Undergraduate Degree Program you are applying and secure the testing permit. 5. Take the Entrance Exam as scheduled.
