Open Registration/Re-registration Period
This year District 91 is offering an open registration/re-registration period for parents to enroll their students. During this period parents will be able to register/re-register students at their chiId's schooI during the hours of 8:00 a.m. - 3:00 p.m. There are also two evening registration dates: Tuesday, August 6, and Wednesday, August 14.
DATES: Monday, August 5, through Thursday, August 15, 2013
TIMES: 8:00 a.m. - 3:00 p.m.
Evening Registration/Re-registration Period
DATES: Tuesday, August 6, 2013 Wednesday, August 14, 2013
TIMES: 5:00 pm - 8:00 pm
LOCATIONS: Field-Stevenson School (Grades 1 - 8) Grant-White School (Grades 1 - 8) Forest Park Middle School (Grades 1 - 8)
Parents can register multiple students in grades 1 - 8 at any school building, however parents with students in grades 6 - 8 only you must register/re-register at Forest Park Middle School.

Designated Attendance Centers
All Forest Park Preschool students (ages 3 - 5) attend Garfield School; contact Mrs. Rice at Garfield School 366-6945 to register/re-register. If you live south of the I-290: SK - 2 attend Betsy Ross School; 3 - 5 attend Field-Stevenson School. If you live north of the I-290: SK - 2 attend Garfield School; 3 - 5 attend Grant-White School. All students in grades 6-7-8 attend Forest Park Middle School.
2013-2014 Book Rental/Material Fees Preschool $100 per year Sr. Kindergarten Grade 5 $35 per year Middle School Grades 6-7-8 $40 per year Plus: $6 Assignment Note Book $3 Calendar/Handbook Fee $20 Gym Uniform (shirt & shorts)
Parents must be prepared at the time of registration/re-registration to pay book rental/material fees.
Beginning August 16, a $25 late fee will be added when re-registering returning District 91 students.
THE DISTRICT WILL CHARGE A $25 FEE FOR ALL NSF CHECKS.
ALL NEW STUDENTS REGISTERING IN DISTRICT 91
Must have the following documents PLUS verification of residency: Official Birth Certificates for each student (Hospital certificates are not acceptable.) Official transfer from the previous school (if applicable) Current physical
VERIFICATION OF RESIDENCY All parents/guardians must provide current documents as proof of residency each year. Documents are required to clearly indicate the names and addresses provided for registration/re-registration of all students.
Parents/Guardians must provide documents as outlined below:
Category A - One (1) document Category B - Two (2) documents Real estate tax bill Gas bill Public aid card/Medicaid card Signed lease Electric bill Driver's license/State D Mortgage document or payment book Water/sewer bill Food stamp card Residency Attestation Phone bill Credit card statement Military housing letter Cable bill Paycheck stub Section 8 letter Bank Statement Village sticker receipt Vehicle Registration
If you have difficulty providing all three documents, District 91 may require a home visit and/or additional documentation to verify residency.

PARENTS/LEGAL GUARDIANS ARE ADVISED THAT THE PROOF OF RESIDENCY INFORMATION REQUESTED WILL BE USED BY SCHOOL OFFICIALS TO HELP ESTABLISH THE ELIGIBILITY OF EACH CHILD FOR ADMISSION TO SCHOOL. FALSIFICATION OF INFORMATION MAY RESULT IN YOUR CHILD BEING EXCLUDED FROM SCHOOL AND MAY EXPOSE YOU TO MONETARY LIABILITY UNDER ILLINOIS LAW FOR PAYMENT OF TUITION. ENROLLING CHILDREN IN A SCHOOL DISTRICT THAT ARE NOT CONSIDERED LEGAL RESIDENTS IS A CLASS C MISDEMEANOR CRIMINAL OFFENSE PUNISHABLE BY A $500 FINE OR UP TO 30 DAYS INCARCERATION.
