ENERGISATION OF S/S 02 (DISTRIBUTION 34.5 Kv) Date: September 02, 2013 During the walk down on 02 .09.

2013 it was observed the following:

1. Comments regarding actual status

1.1. Safety issues observed in SS02: Incoming B cables are disconnected from the panel and free on air, even if SS01 is energized. In this case potentially (safety point) the cable is energized and completely dangerous for people. Battery room was with door not locked, even all battery terminals are exposed due to missing protection covers on terminals and batteries was energized and in service.. Any accidentally short circuit on battery terminals can blow up the battery and generate big injury for people (door open invite workers to stay in air conditioned room); Missing eye wash (even temporary) in battery room. In case of battery damage no way to wash the acid; Access over AC cables & pipes for battery room in SS02 is blocked (is very high). 34.5 KV cable route from SS01 to SS02 is not marked. Safety issues observed in SS01: All outgoing feeders are not earthed by closing Q38 earthing switch. LOTO procedure is not implemented and no LOTO applied. Unacceptable situation was generated for all outgoing the feeders, due to fact that SS01 is energized and all the outgoing feeders without Q38 closed and LOTO applied are considered alive according to standards (OSHA), with real danger. Proposed measures to close(temporary) points 1.1 &1.2: All outgoing feeders earthing switches Q38 in SS01 to be close and LOTO procedure to be implemented; Temporary eye wash to be installed in SS02; Battery room door to be locked by chain and padlock; A bump structure to be installed over the AC pipes on SS02 battery room. Cable route marks to be installed, even temporary.

2. Comments related to condition for energization SS02

2.1. Safety issues to be solved before energisation: Energization should be conducted hot work permit Before energization interlock and inter-trip test should be conducted Schneider Safe operating procedure should be in place All the outgoing cables are in air, without any possibility of protection. Before energisation all earthing switches for outgoing feeders from SS02 and for Incomer B to be closed and LOTO procedure to be implemented, according with Schneider recommendation; In all downstream SS supplied from SS02, incomers line earting switches to be closed and LOTO to be implemented; Incomer A cable to be marked with red/white tape and labeled as energized cable.; Excavation area under SS to be solid barricaded; Housekeeping to be done and all remain cables/drum, which are sited near stairs, needs to be removed; SS number to be indicated in at list to parts (north/south) on temporary boards size aprox. 1 x 1 m; During energisation all the work in SS to be stopped, access to have only designated persons; Road access behind SS to be blocked during energisation to avoid any accident due to eventually blow up of cable in area not backfilled; LOTO procedure to be implemented and authorized LOTO persons in charge to be designated. 24 /24 hours electrical switching persons to be in SS during the test. Hazard communication signs ( no smoking, Dont extinguish with water, Noise hazard, high voltage electrical hazard signs, authorized personnel entry sign) Mandatory PPEs signs ( Electrical Safety Gloves, Coverall, Face Protection) in combination with other required PPEs; Controlled access (locked )and restricted (login/logout) Cordoned off area Fire water tanker/portable Fire Extinguisher 100 kg (outside) Fire fighters Medical team with ambulance on site during energization Means of communication needs to be identified (Radio/ cell phone )
