ONENESS BLESSING GIVER INITIATION COURSE AGREEMENT

Prior to signing this Agreement, I, the undersigned applicant for the Oneness Blessing Giver Initiation described below to be given during the period set forth below (the Course), hereby acknowledge that I have discussed the Course with appropriate representatives of the Oneness Trainer conducting the Course (the Trainer). To further the success of the Course, help ensure the continuation of these and similar Courses, in consideration of my acceptance to the Course, and for other good and valuable consideration, I represent, warrant and agree, as follows: 1. My decision to attend the Course is based solely upon my personal experience thus far with the Oneness Blessing. My attendance at the Course is for the sole and exclusive benefit of my own personal development, and I affirm that I freely and voluntarily choose to participate in the Course. No promises or warranties of any benefits or particular results have been made to me or have influenced me to enter into this agreement or participate in this Course. I shall comply with all Course rules and regulations, and will conduct myself in a manner that will not disrupt the smooth functioning of the Course. I am presently in good mental and physical health, and I am performing well in my daily personal and professional activities. I understand that the Oneness Blessing and any other aspects of the Course are not a substitute for professional medical treatments for physiological or psychological complaints, and do not constitute advice on medical or psychological treatments. I agree that I shall, at my own expense, obtain any appropriate medical treatment that may be necessary while I am attending the Course. If the Trainers leading the Course (the Course Leaders) determine in their discretion that a medical emergency exists, I authorize them to obtain medical treatment for me at my expense. I understand that the Oneness Blessing and participation in the Course do not require the acceptance of any belief or lifestyle. I agree that the Course is an educational process and that maximum benefit is gained when participants freely and fully examine for themselves the ideas presented and evaluate these ideas in light of their own understanding and experience. I agree that the Oneness Trainers offering the Oneness Blessing and the Course are individuals dedicated to benefitting the individual, society and the world, and that all of their resources and energy are used to fulfill these valuable purposes. I desire to ensure that the activities of these Oneness Trainers and the Oneness Blessing Givers who work on their behalf are not hindered by the expense and other burdens involved in defending legal proceedings. I agree to take full responsibility for my participation in the Courses and the results arising from such participation. I agree not to bring any legal actions against these Trainers or Oneness Blessing Givers for any negligent or non-intentional acts or omissions, and I release these Trainers, agents, employees and Oneness Blessing Givers from all such claims and damages, and for all breaches of express and implied warranties (including warranty of merchantability). These Trainers shall be entitled to enforce this provision of the Agreement by injunctive relief as well as be entitled to any other legal or equitable remedy. If the refund policy of this Course entitles me to a refund, I understand that if I do not send a refund request to the office through which I applied by the Course beginning date, the course fee is not refundable. I agree to promptly withdraw from the Course if for any reason the Course Leaders decide, in their sole discretion, that such withdrawal is advisable. I understand that the course fee will not be refunded in case I am asked to leave the Course because of my mode of conduct or in case I leave the Course without the consent of the Course Leaders. In case I have to leave the Course for reasons of health, I understand that it will be possible to complete the Course only if the same course is available at some other time and location, and only if it is considered desirable according to the judgment of the Trainer conducting the Course. I will be informed by the Trainer if and when the opportunity to complete the Course arises. I represent that everything that I have expressed in my Course application and in this Agreement is accurate and true, and I agree to compensate and indemnify the Trainers or Oneness Blessing Givers involved in any way with the teaching or contents of the Course for any loss or damages, including attorneys fees, and

other expenses that may arise from any false representations made in this Agreement by me, or from my breach of any other provisions of this Agreement. I further agree that any violation of this Agreement by me may result in the immediate termination of my participation in courses or programs offered by the Trainer or its affiliates. The representations, warranties and agreements that I have made in this Agreement may be relied upon and/or enforced not only by the Trainer offering this Course but also by all affiliated Oneness Trainers and all Oneness Blessing Givers and Trainers that offer the Oneness Blessing or Oneness Blessing Giver Initiation. 7. MEDIATION AND ARBITRATION: If any issue or claim arises between the parties, both agree to seek in good faith to resolve such issue or claim through negotiation using, if necessary, the assistance of a third party mediator who shall be chosen by Andy Migner.

If the above procedure does not resolve the issue, both parties agree that any issue or claim that is related in any way to this Agreement, or to which the terms of this Agreement may apply, including the validity of this Agreement, shall be settled by arbitration in Ma., on a confidential basis, in accordance with the laws of that state and rules that the arbitrators may adopt. Three arbitrators shall be chosen by Andy Migner. These Trainers shall have successfully completed at least a Level 1 Oneness Blessing Giver Process, and shall not, within the year prior to being chosen as arbitrators, have had professional or business relationships with Andy Migner. I understand that this appointment procedure applies even if Andy Migner is a party to the issue to be arbitrated. The decision or award of the arbitrators, including awards of provisional relief, may be enforced in any court and the parties consent to the exercise of jurisdiction over them in Ma. Both parties agree to equally share in the costs of the arbitration process. 8. I agree that this Agreement can only be changed by a written modification signed by both parties. If any provision of this Agreement is found to be invalid, this shall not affect the validity of the remaining provisions. I hereby agree that I may not assign my rights and obligations under this Agreement to any party. I have read and understand everything in this Agreement, and I intend to be legally bound by this Agreement.
