A. THE FAMILY UNIT. We have a nuclear type of family. Only the three of us are living in our place.

However, my relatives on my fathers side are living in the same city, the same area where my paternal grandparents used to live and thus we have a patrilocal residence. Our family belongs to the middle class. INTERPERSONAL RELATIONSHIP The family has an open type of structure. There is no apparent external boundary and everyone is encouraged to interact with the community. Our family exercises a democratic home environment. Each ones side is heard whenever conversations occur. In addition, an equalitarian type of decision making is practiced. My parents share more or less equal percentage in terms of making decisions. They consult each other before one act. However, my mother is more of a disciplinarian than my father. My father most of the time is the one preparing our daily meal since my mother is busy at school as a grade school teacher (aside from the fact that my father is a better cook than my mother) FAMILY ROLES My father is running a small diner nearby the sea just a few hundreds of meters from our house. He is the one in charge for our daily meal budget. My mother works as a science class teacher in grade III at Dapitan Central School, the same school where I finished my grade school though she was just transferred there few years after I graduated. She is the one in charge of paying our house bills and sustaining my monthly allowance here in Med school. The three of us usually join forces when doing the household chores. Sometimes, we hire somebody to help us in doing our laundry and at times when we have our general cleaning. But most of the time, we carry the tasks by ourselves.

COHESIVENESS STRESS it is quite a big word but still it is way more fragile compared to our faith in God and bond with each other. Whenever our family has to come up with a major life decision, we all talk about it. No one acts without the knowledge of all. We make sure that whatever one does, all of us are informed for support and if needed, for enlightenment whether the decision that has to be taken is more beneficial than threatening. Sometimes, my mother supports my father. At times, when my father feels really down, my mother balances everything. She is the most religious of the three of us. And this may be the reason she serves as our guiding path and enlighten our minds when at times that there is a threatened shake in our family status. I could say my family is great. My parents really want to live a peaceful and simple life. As much as possible they dont want to have misunderstanding with the people surrounding us. For me, they are too good to the extent that sometimes other people abuse it. Whenever my parents detect that something is going wrong, they just try to avoid it if not fix it by themselves even if they are not the one causing it. They just dont want that our home will be disturbed. Perhaps, prayers and faith in God that everything will turn out fine and those things are just trials and lessons to be learned are the fortresses of our family. COMMUNICATION

We have constant daily communication. At home, during each meal especially dinner, we take time to converse with each other and update one another in terms of our individual activities. There is open communication among the three of us. Right now, while I am here in Luzon, our major form of communication is texting and calling each other. ETHNIC AND CULTURAL BACKGROUND Both my parents descend from a family where superstitions and traditional medicines are with value. For instance, they try native or herbal remedies first before resorting to pharmaceutical drugs for simple illnesses. ORGANIZATION AND DEGREE OF COOPERATION I dont have siblings. My family is very supportive. We support one another. Whenever one does not agree with the other, we talk about it and resolve to come up with the better if not the best idea. SOCIAL ACTIVITIES OUTSIDE THE FAMILY My family is an active member of the church organizations and participates in religious activities. My mother is involved in the committees in their school. My father is a member of the Senior Citizen Club in our city. I am a member of a local performing troupe and youth organization in our city. RELIGIOUS HABITS All of us are Roman Catholics. We go to church together especially on special occasions. My family participates in the Dawn Rosary. RECREATIONAL PATTERNS Our favorite past time is watching TV. Also all of us are fond of cooking. Thus, we help each other and have our bonding when preparing our meals or snacks. My mother plays computer games sometimes and plays badminton with her friends. I most of the time watch movies on DVD and hang out with friends. OVERALL FAMILY STRENGTHS AND WEAKNESSES The family that prays together stays together. I could say that I have a strong family since it is rooted in Gods faith. Our strong belief in the Divine Providence is the one that supports and binds us especially at times of weakness and stress. About my familys weakness, perhaps it would be being apart from each other. However, it is compensated by our constant communication. NUTRITIONAL HABITS Ever since, my parents practice having diet with vegetables each meal. Perhaps the most constant food in our dining next to rice is a dish of veggies. Also, fruits are always present on the table. Since we live near the sea, my father always buys fresh fish and other seafood. We have an adequate balanced diet every day. Breakfast and dinner are a family meal-together. Sometimes we have lunch separately due to work. MOBILITY

My family has never transferred residence since I was born. B. FAMILY HISTORY There is history of cancer particularly prostate cancer and leukemia in my paternal grandparents. Hypertension is a common disease among my paternal siblings. There is no case of possible heredofamilal diseases in my maternal side except from the hypertension that my mother has. C. D. ENVIRONMENTAL ASSESSMENT Home Description Sanitary standards

There are two flush-type toilets: one is in the masters bedroom while the other is located near the kitchen. The kitchen has two sinks and is equipped with the basic appliances such as rice cooker, refrigerator, gas range, and blender. The tableware is kept in an enclosed container while the other utensils are stored in the cabinet. The main water supply is from NAWASA, our local water provider, while we purchase our drinking water from a nearby water refilling station. The house has a proper sewage pipe system which is directly emptied into the city drainage system. The garbage is collected twice a week by the local government. Safety standards Basic first aid materials and medicines are kept in a wall cabinet. Electric wires channeled in tubes for it not to be exposed. The gas tank is kept inside a cabinet under the sink. To date, there is no apparent potential source of accidents inside the house. Sleeping Arrangements The house has 3 separate rooms. My parents stay in the masters bedroom, which has its own toilet, while I stay in the room next to their room. The last room serves as the storage room for the clothes though it has also a queen-sized bed to rest on. My room is about 3-5 meters away from the main toilet. Pets My family loves dogs. Our pet dogs are not only our best friends but they are also our house guards whenever we are away. We have four dogs in all. Description of neighborhood Each house in the neighborhood has its own way of privacy. It is separated by fences or walls. Each house has its own front yards/ gardens. The passageway is clean and concrete. The main roads are paved with proper drainage system. The surrounding is not noisy. It is conducive for sleeping at night. Distance to nearest Medical Facility The nearest hospital is a secondary hospital, Dr. Jose Rizal memorial District Hospital. It is less than 2 kilometers away from our house. The nearest health center is located at the center of the city near the city plaza which is about half a kilometer away from our home. Distance to schools, shops Our house is located less than half a kilometer away from the market place as well as from the grocery store. It is just a 10-minute walk from the house. The nearest school is just nearby. It could be seen while on the way to the market. Relationship to neighbors We have a friendly neighborhood. Especially during Christmas season, we have a small gathering for everyone. E. ECONOMIC BACKGROUND

Source of income Our main source of income is my mothers salary as a grade school teacher and from the income of my fathers business. My aunt also gives us money especially now, to support my studies. Method of financing healthcare We are covered by the Philhealth. Also, from the monthly income, part of it is allocated for financing healthcare or we just look for money when the need arises. F. ATTITUDES TOWARDS HEALTH CARE Methods of providing medical care In times of common illnesses such as simple fever, cough and colds and minor traumas, we just self medicate. My family is also practicing the use of herbal medicines. In more serious illnesses such as my parents hypertension, we consult our family friend who is a doctor. Preventive measure My parents have their yearly flu vaccination and pneumococcal vaccines every 5 years. As part of the school requirement, my mother has her annual physical exam. Availability of care There are no problems in terms of accessibility and availability of health care. Dental care I have my monthly dental check up. My mother has her dental check up 1-2 times a year. Health awareness My father is a smoker. He is aware of the consequences of smoking though he still has not fully stopped smoking. He smokes occasionally nowadays. Family planning Unfortunately, this method is very powerful in our familys case since I am the only child. Attitudes towards children My parents are very loving and considerate. If they think one thing can do me good, the permit it. But if not, they make sure that they protect me. They are very open and understanding. They make sure that every happening will always be better if not best.

He has undergone 6-month antiKochs therapy and was TB-free after the treatment. No previous hospitalizations. No surgical operations. No regular medical consultations done. Has maintenance antihypertensive drug Captopril once a day. Allergies: No food and drug allergies. Social history: He is a high school graduate. He used to work as a carpenter. Currently, he has a small diner by the sea. Habits: He smokes a maximum of 5 sticks per day. He used to smoke a pack of cigarette when he was younger (starting at 20 years old) but tapered his consumption since 2000 up to present. He is also an occasional alcoholic drinker. He consumes an average of a liter of beer per occasion. No history of illicit drug use. He does brisk walking early in the morning daily from our house to the workplace which is less than half a kilometer away. He regularly practices low salt, low fat diet and is inclined to fish and vegetables most of the time. Medications: Prescribed Captopril for BP control; Self-medicate with oil liniments for body aches, Bioflu for fever and colds. Hobbies: Cooking would be his main hobby. He is found in the kitchen most time of the day. At night after dinner, he watches with us TV shows.

She does monthly breast self-exam and has annual physical examination. Has maintenance antihypertensive drug Captopril once a day. Allergies: No food and drug allergies. Social history: She is a graduate of Bachelor of Science in Elementary Education. She is currently working as a science class grade school teacher. Habits: She is a non-smoker. She is also an occasional alcoholic drinker. She consumes an average of three glasses of beer per occasion. She used to drink a shot of red wine every night but instead, she now drinks a glass of milk before going to sleep. No history of illicit drug use. She plays badminton and does brisk walking about 1 kilometer late in the afternoon two to three times a week. She regularly practices low salt, low fat diet and is inclined to fish and vegetables most of the time. She eats fruits every after meal. Medications: Prescribed Captopril for BP control; Self- medicate oil liniments for body aches, Mefenamic acid for pain. Hobbies: Playing computer games and watching TV is her hobby when at home. She also plays badminton. She also loves gardening.

Francis Ed M. Gahuman, 25-male, single (Only child- ME) Present illness: Prehypertension Past Medical history: Had two prior hospitalizations both because of Dengue Fever (1996 and 2005). No surgical operations. Allergies: No food and drug allergies. Social history: I finished Bachelor of Science in Nursing at Silliman University and am now a registered nurse. I used to work as an ER nurse at a Tertiary hospital in our province for about a year. I also had experienced working at a call center for 3 months before I studied Medicine in 2011. Habits: I am a non-smoker. I am an occasional alcoholic drinker. I usually prefer brandy and could consume 5-10 shot glasses per occasion. No history of illicit drug use. Walking daily to school is the mere exercise that I do. I used to go to the gym but I stopped due to the hectic schedule in Med studies. I usually eat mostly pork and chicken on weekdays. I only eat fish and vegetables during weekends. Medications: Self-medicate with Bioflu for fever and colds. Hobbies: Watching TV, listening to music, going out with friends. I used to cook when I am at the province.
