AILURE MODE (Cause of failure)

To insulate between :onductor and support structure at rated line voltage

Uot capable of insulating letween conductor and support structure at rated voltage at all.

Contamination bv salt leadina to electrical erosion of sheds o n polymer compisite insulators.

FAILURE EFFECT (What happens when it fails)

I Reduction in insulatina caoacitv. Probabilitv that wood

structures will catch fiye with A t a g e injectibns and loss of supply. Concrete and steel structure have potential to become alive resulting in public safety hazard & loss of supplyAt time of this RCM there are no known failures recorded within Powercor. Damage has been identified in Powercor but effects have not been determined. Damage so far has been in terms of appearance, with no reported cases of mechanical or electrical failure resulting. Damage has not been identified in Powercor & effects have not been determined, other than obvious sites where the resultant damage causes power outage & patrol of line identifies the site. Flashover of current acrosslthrough bird or animal at insulator. This may result in line outage due to protection operation, and should animal I bird fall to ground this may result in potential for fire start.

Damage to polymer insulator by birds chewing the sheds.

Electrical damage by vandalism or lightning.

Insulator bridged out by bird or animal.

mec:hanically retain the :onductors to the main support structure

Jnable to mechanically retain he conductors to the main support structure*

Conductor tie I termination failure.

Conductor comes loose from insulator and can result in reduced conductor clearances, or pole fire. At time of analysis there had not been issues with this failure mode within Powercor. As per 2A1

Mechanical damage by vandalism or lightning Insulator manufacture defect
