ACCOMPLISHMENT REPORT

SY: 2011 2012

A. STUDENT DEVELOPMENT
1. Classes for the SY started June 06, 2011. Orientation of classes was done wherein the teachers discussed the grading system, school policies and introduction of the course and requirements of the subjects. 2. The school diagnostic test was taken by all students last June 7-8, 2011 to determine the baseline of each student in the different year levels. 3. Performed some of the physical fitness and sports talent tests. 4. All SPA students prepared presentations for the visitation of Under-Secretary Alberto Muyot last July 05, 2011. 5. MAPEH students in all year levels accomplished their outputs, projects and practicums on time. 6. MAPEH students were actively participated in the class demonstration teaching. 7. The school choir CORO EULOGIANO with Mr. Jowell Oaa led the doxology and in singing the National Anthem of the Philippines during the celebration of the Buwan ng Wika last August 25, 2011 and the Division Celebration of the Buwan ng Wika held at Don Eulogio de Guzman Memorial National High School. 8. MAPEH students in all year levels finished their first periodical test with good results. 9. The school choir CORO EULOGIANO with Mr. Jowell Oaa led the Pambansang Awit and entertain the visitors at the Provincial Capitol, San Fernando City, La Union last August 26, 2011. 10. MAPEH students in all year levels accomplished their outputs, projects and practicums on time 11. MAPEH students were actively participated in the class teaching. 12. Coro Eulogiano with Mr. Jowell Oaa led the Doxology, Pambansang Awit and one intermission number for the 2011 Regional Recognition and Awards Rite during the 111th Philippine Civil Service Anniversary at Hotel Ariana, Paringao, Bauang, La Union last September 16, 2011. 13. Second year students joined the Inter-Section Basketball Tournament held at Don Eulogio de Guzman Memorial National High School. 14. Coro Eulogiano performed at the Provincial Capitol, city of San Fernando during the Regional Statistics Closing and Awarding Ceremony last October 24, 2011. 15. Coro Eulogiano sang the doxology during the (UPF) Universal Peace Federation Gathering at the Bauang Farmers Hall last November 28, 2011.

B. STAFF DEVELOPMENT
1. Serves as a musical director during the annual Recital of the Special Program in the Arts held at the Covered Court of Don Eulogio de Guzman Memorial National High School, Calumbaya, Bauang, La Union. 2. Conducted the school choir CORO EULOGIANO in the singing of the doxology and National Anthem of the Philippines during the JS Promenade of Don Eulogio de Guzman Memorial National High School held at the Sobrepaa Civic Center, Caba, Bauang, La Union. 3. Mr. Rodel Rimando, Mr. Jowell Oaa and Mr. Marion Christopher Corpuz undertook the TIPTeacher Induction Program with Mrs. Lorna O. Gaspar as their instructor. 4. MAPEH teahers attended the Unit II Athletic Meet held at Bacnotan National High School last November 17-18, 2011. 5. MAPEH Teachers participated during the Panagkakararua 2011 where they went 3rd placer held at the Bauang Farmers Civic Center, Bauang, La Union on October 31, 2011. 6. Conducted the School Choir CORO EULOGIANO during the 2011 Regional Recognition and Awards Rites in the 111th Philippine Civil Service Anniversary held at Hotel Arianna and Restaurant, Paringao, Bauang, La Union. 7. MAPEH teachers attended the North Luzon Dance Festival: Ba-ile at SM City Rosales, Rosales, Pangasinan as coached of the Taddek Dance Troupe and won the Peples Choice Award (First Price) and 1st Runner-up on the Interpretative Dance Contest last September 4, 2011.

8. Attended the Universal Peace Federation Seminar at Provincial Capitol, San Fernando City, La Union last August 26, 2011. 9. MAPEH teachers were observed during their class demonstration teaching. 10. MAPEH teachers were observed during their class demonstration teaching. 11. Five (5) SPA teachers prepared their students for the visitation of Under-Secretary Muyot last July 05, 2011. 12. Conducted the school choir CORO EULOGIANO during the seminar on Good Governance spearheaded by the Ombudsman held at Regional Education Learning Center, San Vicente, San Fernando City, La Union last December 5, 2011. 13. MAPEH Department took part during the Christmas Booth Contest spearheaded by the Values Education Department from December 12-19, 2011 and won the 1st Prize at Don Eulogio de Guzman Memorial National High School.

C. CURRICULUMN DEVELOPMENT
1. Had prepared and used audio-visual materials, drills and exercises to enhance teaching and learning instruction. 2. All MAPEH teachers responsively attended all departmental meeting. 3. Teachers had prepared test questions with table specifications for the diagnostic test, checked and evaluated. 4. Prepared daily lesson plans based from the CBIS, learning plans based on the Learning Guide and other DEPEd Forms.

D. PERSONALITY AND HUMAN RELATIONS

1. Observed the rules and regulations affecting public services and standard of morality. 2. Had developed sense of responsibility, initiative, cooperation, honesty, and benevolence with fellow mentors. 3. Maintained harmonious relationship with other people. 4. Exemplified the value of love and respect to others.

E. COMMUNITY AND ALLIED SERVICES

1. Joined necrological services. 2. Mr. Jowell Oaa and the SPA Music Arts majors serve as choir at St. Joseph the Worker Parish at Santiago, Bauang, La Union every second Sunday of the month. 3. Mrs. Noriville Busto had a home visitation of her students at Payocpoc Sur, Bauang, La Union last August 09, 2011 and at Parian Oeste, Bauang, La Union last August 13, 2011. 4. Coro Eulogio with Mr. Jowell Oaa as conductor sang during the Misa de Gallo at St. Joseph the Worker Parish, Santiago, Bauang, La Union last December 18, 2011 at 7:00 in the evening with Mrs. Noriville Busto.
