1. Performed hand hygiene and applied gloves. 2. Perineal Care for the Female a. Observed patient's ability to maneuver and handle washcloth. Allowed patient to clean perineum on own. b. Assisted patient to dorsal recumbent position. Placed waterproof pad under buttocks. c. Draped patient for privacy. d. Folded lower corner of bath blanket up between patients legs onto abdomen. Washed and dried patients upper thighs. e. Washed labia majora. Wiped in direction from perineum to rectum. Repeated on opposite side using separate section of washcloth. Rinsed and dried area thoroughly. f. Gently separated labia with nondominant hand to expose urethral meatus and vaginal orifice. Washed downward from pubic area toward rectum in one smooth stroke. Used separate section of cloth for each stroke. Cleansed thoroughly around labia minora, clitoris, and vaginal orifice. Avoided tension on indwelling catheter if present, and cleaned area around it thoroughly. g. Rinsed area thoroughly. If patient used bedpan, poured warm water over perineal area. Dried thoroughly, using front-to-back method. h. Assisted to a comfortable position. 3. Perineal Care for Male a. Observed patients ability to maneuver and handle washcloth. Allowed patient to cleanse perineum on own. b. Assisted patient to supine position. c. Folded lower half of bath blanket up to expose upper thighs. Washed and dried patient's thighs.

S d. Covered thighs with bath towels. Raised bath blanket up to expose genitalia. Raised penis and placed bath towel underneath. Grasped shaft of penis. Retracted foreskin if present. Deferred care if patient had an erection. e. Washed tip of penis at urethral meatus first. Using circular motion, cleansed from meatus outward. Discarded washcloth and repeated with clean cloth until penis was clean. Rinsed and dried. f. Returned foreskin to its natural position. g. Cleansed shaft of penis and scrotum by having patient abduct legs. Paid special attention to underlying surface of penis. Lifted scrotum carefully and washed underlying skinfolds. Rinsed and dried thoroughly. h. Folded bath blanket back over patients perineum and assisted patient to comfortable position. 4. Observed perineal area for signs of persistent irritation and redness. 5. Disposed of gloves. Performed hand hygiene.
