All should be considered. RESPONSE (HAZMAT RESPONSE) ITEM # TASK/ACTION OPR All Minor One pint or less. Major More than one pint. 1. Establish command IC 2. Request air sampling assistance from the BEE for a hydrazine leak. IC 3. Establish a cordon including Hot, Warm, and Cold Zones as needed. IC Clearly mark the boundaries between the zones. Clearly identify ingress and egress points after the zones are established. NOTE: See Figure 1, Typical On-Scene Incident Setup. 4. Authorize access to the cordon and ensure appropriate personal IC protective equipment (PPE) is worn. 5. Establish tactical priorities. IC Ensure rescue/life safety/responder safety. Determine presence/absence of contamination. Identify boundaries of contamination. Stabilize the incident. Conserve property & the environment. 6. Develop an incident action plan. IC/ CEF/ BEE/CEX/ CEV Identify problems. Examine conditions surrounding the problems. Develop possible solutions. Evaluate the alternatives. Choose best option. Implement the plan. Implement actions described in the (YOUR INSTALLATION) HAZMAT Emergency Planning and Response Plan. For hydrazine leaks consider using the maintenance hydrazine response team. Ensure protective measures for personnel working at the incident scene. Appropriate training. Appropriate level of PPE according Occupational Safety and Health Administration (OSHA) regulations. Work-rest regimens. Protective measures against climatic conditions. Food and water. Sanitary facilities. Procedures to monitor stay times for entry teams. Occupational and environmental surveillance and health risk assessment. Personnel exposure levels. Forward numbers of Dead, Injured or Missing to the EOC as the information becomes available. Relay casualty information to the EOC. Forward names of dead and injured to EOC by runner. o Do not use cell phones. Ensure only medical authorities certify death. Caveat reports of injured and dead with believed to be when identification cannot be confirmed 100%. Provide alert photographer and ensure access to the scene when safe to provide official documentation of the accident. Planned Withdrawal Declare after all firefighting, rescue and containment actions are completed. Immediate Withdrawal Declare emergency withdrawal over radio/public address systems. Sound vehicle mounted sirens. Withdraw in an upwind/crosswind direction and take immediate cover inside the cordon area no closer than 300 feet from the ECP. Inform wing leadership of withdrawal and status of response forces. Authorize re-entry when all clear is given Terminate the incident 5

All should be considered. SHELTER IN-PLACE PROCEDURES Actions for Personnel Outside of Buildings ITEM # TASK/ACTION 1. Take cover to protect from fire, explosive hazards, falling objects, and inhalation hazards. 2. Notify other personnel in the vicinity, don available protective equipment, and evacuate the area by moving in an upwind or crosswind direction away from the point of release (if visible). 3. Report the incident to the FACC or law enforcement SFCC, call 911,if emergency response forces are not at the scene. 4. Move to a designated unit assembly point or location (if not located in the hazard area). 5. Perform self-aid and buddy care as needed. 6. Do not drive vehicles unless directed by emergency response forces or unless required to evacuate injured people or those that cannot walk. If you are already in a vehicle, roll up windows, turn-off the ventilation system, and attempt to safely exit the hazard area. 7. If you cannot evacuate the area, go to the nearest building and take cover inside. 8. After evacuating or sheltering, contact the appropriate C2 echelon; advise of the location and situation. 9. Remain in shelter or at the assembly area and continue to wear protective equipment (if available) until directed otherwise by unit or emergency response personnel. SHELTER IN-PLACE PROCEDURES Actions for Personnel Inside of Buildings -External Hazardous Material Release TASK/ACTION Take cover if needed to protect from fire, explosive hazards, falling objects, and inhalation hazards. Don available protective equipment. Notify other personnel in the vicinity. Call 911 and report the incident to the FACC or SFCC if emergency response forces are not at the scene. Close and lock windows, vents and fireplace dampers. Turn off heating, ventilation, and air conditioning (HVAC) systems and exhaust fans. Implement shelter in-place procedures. Move to a designated safe room, if available. Seal room with plastic and duct tape. Seal door threshold with a wet towel. Stay away from outer walls and windows. Do not use any items with open flames. 7

Lock up or secure classified material and funds if possible. Evacuate the building if directed by the ICC, EOC or UCC or emergency response forces. If directed, evacuate by moving in an upwind or crosswind direction away from the point of release (if visible). Move to the designated unit assembly point or location. Do not drive vehicles unless directed by emergency response forces or unless required to evacuate injured people or those that cannot walk. Contact the ICC, EOC or UCC and advise them of the location and situation. Remain in shelter until directed otherwise by the unit or emergency response personnel. Do not allow entry or exit during the plume passage. However, remain alert for personnel seeking entry into the building to seek shelter from the incident. Allow entry if identification is confirmed and the situation does not present a high risk to others within the building. Purge the building when notified that the outside hazard has passed. SHELTER IN-PLACE PROCEDURES Actions for Personnel Inside of Buildings -Internal Hazardous Material Release. TASK/ACTION Take cover if needed to protect from fire, explosive hazards or falling objects. Don available protective equipment. Notify other personnel in the vicinity. Report the incident to the FACC or law enforcement desk if emergency response forces are not at the scene. Turn off the building HVAC systems and exhaust fans. Perform self-aid and buddy care as needed. If possible, close doors and windows within the release area to contain the hazard. Lock up or secure classified material and funds if possible. Evacuate the building. Once outside, evacuate by moving in an upwind or crosswind direction away from the building. Move to a designated assembly point or location (if not located in the hazard area). Do not drive vehicles unless directed by emergency response forces or unless required to evacuate injured people or those that cannot walk. After evacuation, contact the ICC, EOC or UCC and advise them of the location and situation. Remain at the assembly area until directed otherwise by the unit or emergency response personnel.

Under normal circumstances the Monitoring Division of the Federal All Radiological Monitoring and Assessment Center (FRMAC) will collect radiation samples. (DOE/NV/11718-181, FRMAC Monitoring and Analysis Manual) 1. Assemble the sampling team. IC/BEE/CEX Do not conduct operations in a contaminated area without backup personnel present. They should be partially dressed in the appropriate PPE and prepared to enter the area as an emergency extraction team. The size and composition of the sampling team may vary based on the size, type, and scope of the incident and the response. At a minimum, the team should consist of five personnela team leader, a two-person sampling team, and a two-person backup sampling team. 2. Plan the actions of the Sampling Team. IC/BEE/CEX Sketch the layout of the area where sampling is to take place. The layout of the room/field should be documented with dimensions, measurements, and items that might be of interest. Relevant reference points, control areas, and hazards should be annotated on the form. The diagram/sketch form can be used to brief the sampling team, which may use the form when conducting sampling operations. Some considerations include: Chemical samples. Chemical samples may include very toxic material. Packaging may be further complicated by the fact that munition fragments or other material (solid or liquid) may be included as part of the sample. Biological samples. Biological samples may include living microorganisms. Keeping the sample at the proper temperature during evacuation is essential to maintaining agent viability. Due to the perishability of some biological material, timeliness is also critical when transferring a biological sample. Quality packaging is a key factor to prevent spillage or breakage during transport. Radiological samples. Radiological samples present further challenges. Packaging and transporting samples require key radiation safety considerations (such as shielding distance and time) to minimize or preclude the occurrence of a hazard. The procedures for radiological-agent sampling are generally the same for CB sampling; however, CB sampling containers do not provide sufficient shielding to protect individuals who are carrying them. o If the primary radiological hazard is residual radioactive contamination, the sampling team could take very small quantities of low-activity environmental samples. o Very small quantities of these samples could perhaps be safely contained in CB sample containers. o The radiological hazard from these samples should be limited to alpha- and beta-emitting radionuclides. Consider physical and chemical characteristics of samples when selecting a suitable container for shipping and sampling. Ensure common sampling procedures are used to ensure quality. Use properly sized and prepared containers with an airtight seal. Keep the empty containers in a clean bag or box. Open the sample container only to add the sample. Use the proper tools to collect samples. Ensure that sampling equipment is dispATCle or that enough clean spares are available to allow single use during a sampling mission. They are only to be considered if a CBRNE threat exists. CBRNE FPCON ALPHA: Measure CBRNE ALPHA 1. Establish/review utilization plan for all hand-held CBRNE detectors (deployment and home station), if available, that will be used at home station during periods of increased threat for terrorist use of CBRNE materials. Measure CBRNE ALPHA 2. Review shelter-in-place and evacuation plans for all facilities on base or controlled by USAF personnel/units Measure CBRNE ALPHA 3. If updates from local medical care providers are not automatically received through the medical surveillance system, contact local hospitals and establish lines of communication for notification of the installation in the event of significant increase in antibiotic use or people exhibiting symptoms of exposure to CBRNE materials. This includes the process for determining where the JIC will be located (on base, off base), and the accompanying operational procedures considering the base will probably be locked down as part of the FPCON Delta measures. Ensure that DRF and if permissible, local authorities are briefed on potential CBRNE threat. Measure CBRNE CHARLIE 14. If the Homeland Security Advisory System is Orange (High) or Red (Severe) determine (through MAJCOM and HQ USAF) if a Principal Federal Official (PFO) has been designated and an Emergency Response Team (ERT) is in the process of establishing a Joint Field Office (JFO). As appropriate, make contact and begin coordination for influx of forces and equipment as necessary. Measure CBRNE CHARLIE 15. Accomplish CBRNE contamination avoidance measures to the maximum extent possible IAW EMP 10-2 and CE CRP. Measure CBRNE CHARLIE 16. Distribute instructions for individual response to CBRNE incidents to the base populace through existing channels. Measure CBRNE CHARLIE 24. Consider issuance of CBRNE pre-treatment supplies, biological prophylaxis, and nerve agent antidotes to the installation population, if credible threat. CBRNE FPCON DELTA: Measure CBRNE DELTA 1. Take appropriate action to finalize/complete all previous CBRNE measures not already accomplished. Measure CBRNE DELTA 2. Initiate enhanced medical surveillance procedures; include triage outside the medical treatment facility. Measure CBRNE DELTA 3.
