Candidates must bring their original testimonials related to Qualification, Date of Birth, Experience, Pass in Punjabi in Matriculation, Reserve Category certificate, Address Proof and Photo Identity Proof ( Copy of Ration Card, Voter Card, Passport, License etc.) failing which their candidature may stand cancelled.

Candidates must bring 2 latest passport size photographs at the time of interview If any candidate is found not to be fulfilling the eligibility/selection criteria at the time of interview or any point of time in the entire recruitment process by the screening or selection committee, his/her candidature will be cancelled/ rejected for the said post.

The candidature of the candidate shall be provisional till the time of verification of documents of the candidate at the time of interview. In case the eligibility of the applicant is not approved by the selection committee on any grounds, his/her candidature shall stand cancelled.

Candidates will not be paid any TA/DA for attending the above said interview. Mission Director, National Health Mission, reserves the right to cancel amend/ change/modify the entire recruitment process at any stage without issuing any notice.
