PSYCHIATRIC PERFORMANCE EVALUATION TOOL Instruction: check on the column of your choice as to what level is the performance of the student in the following procedure. Be guided in the scoring with the legend below: Legend: Level of performance 5 Excellent (outstanding and highly commendable output and response) 4 Very satisfactory (very sufficient and adequate output and appropriate response) 3 Satisfactory (acceptable and adequate output and appropriate response ) 2 Good (insufficient output and incomplete response) 1 Fair (major errors of output and no correct response)

COMPETENCE STANDARS I. Problem Solving Ability( 72 points) A. Assessment (16 points) 1. Collects overt manifestation of patient appearance, behaviour, thoughts and mood with maximal use of sensory organs. 2. Assigns meaning data gathered and classifies them appropriately according to categories in the mental status exam. 3. Validates interpretation made through the use of the following patient, patients family, staff and personnel. 4. Identifies covert and overt problems areas based on NPI B. Interpretation(13 points) 1. Interprets patients behavior using theoretical knowledge and with evidence of thorough research. Formulates the psychodynamics of patients behavior. C. Intervention(13 points) 1. Formulate SMART goals for nursing care based on: a. Identified patients problem areas b. Identified patients assets and abilities c. Formulated psychodynamics of patients behaviour. 2. Uses self therapeutically a. Recognizes own feelings and concern b. Recognizes recognition in formulating and implementing plan of care on the spot interventions and alternative interventions. 3. Uses therapeutic communication techniques Ability to listen therapeutically and allow patient to vent his feelings for catharsis and for data gathering. a. Effects mutual change of ideas and feelings at the verbal and non-verbal level. 4. Identifies and implements interventions which are realistic, individualized, and sustainable. 5. Uses available resources in planning and implementing nursing interventions. D. EVALUATION(12 points) 1. Identifies overt and covert forces operating in the situation which led to outcomes of the intervention. 2. Evaluates Nursing interventions based on the extent to which goals are achieved. 3. Makes modification in plans and communication techniques based on results of evaluation.
