Contract for External Supervision

This is to confirm that <name of supervisor> is contracted to provide clinical supervision to <name of supervisee> on behalf of <name of employing organisation and department>. The supervisor shall be expected to provide supervision of the supervisees clinical work for this organisation with the aim of enabling best clinical practice. Supervision sessions will occur <specify frequency>. Each session will last for <specify duration> Duties of the supervisor The supervisor shall be accountable to <name of designated manager> and shall provide either a brief written report or meet with the manager to report on the progress of the supervision at <specify time intervals >. In this report the supervisor will give feedback about the supervisees practice, maintaining confidentiality about the content of the clinical material, but commenting on the structural and quality aspects of the work, paying particular attention to anything which appears to be interfering with best practice. The content of the supervision sessions shall remain confidential unless the supervisor becomes concerned about the safety of patients, the organisation or the supervisee. In addition the supervisor will notify the manager immediately if there are reasons for concern about the work of the supervisee. Further, if the supervisor becomes aware, through material presented during supervision, of aspects of the functioning of the organisation, which are seriously endangering the clinical work or the well-being of patients, the supervisor will also draw this to the attention of the manager.

The supervisor will discuss any issues of concern with the supervisee before raising those concerns with the employing organisation. Duties of the manager. The manager will provide the supervisor with a copy of the job description, or a detailed description of the salient duties of the supervisee and will undertake to notify the supervisor of significant changes in those duties. The manager will also undertake to inform the supervisor of any concerns about the supervisees work as they occur.

This contract will commence from <specify start date> and will be reviewed at <specify date for reviewing the contract>. The review will include the following: a re-appraisal of the roles and duties of the supervisee

a review of the frequency and duration of sessions the frequency of reporting arrangements fees duration of the contract setting the next review date

The fee for this work will be <specify hourly rate or agreed fee>. Invoices are to be sent to <specify>, listing dates of sessions. Sessions cancelled by the supervisee will/will not be paid for, <specify whether there is an acceptable period of notice of cancellation>. Sessions cancelled by the supervisor will either be re-scheduled or not charged for.

Arrangements for terminating this contract This contract can be terminated by the supervisor, manager or supervisee, with at least one months notice being given by the terminating party. Signatures:
