Quick Guide to Corrective Action

A good Corrective Action should:


Already be implemented Include documentation (for training, work orders, etc) Address the root cause of the problem Should not be written in future tense

What is?
Corrective Action: Action taken to prevent recurrence and eliminate the root cause of a detected nonconformity(-ies). Nonconformance: Condition in which one or more characteristics do not conform to requirements. Root cause: Underlying reason or cause for a nonconformance.

BAD Corrective Action
During machining process, operators should ask if they see something different or odd about sales order instead of ignoring or assuming order is correct.

Operators have been instructed to clarify sales orders if they are not clear before commencing work. Training was performed on December 18, 2006 to all operators at xx facility. See attached training document. ZE ###### has been created and completed, revising Drawing X-###### to clarify the desired machining and thus reduce the possibility of incorrect parts. Training held on December 18, 2006 with XXX machine shop, inspection, and assembly floor personnel to watch for XXX defects. See attached training document. ZE ###### has been created and Engineering in location is working on designing specific gages for new production, etc and for multiple sizes. A special test procedure (X-#######) was created and approved by XXXX and XXXX to verify that all part ###s would effect a seal prior to shipment. This test procedure was implemented December 18, 2006. Training was completed the same day. See attached documentation.

What is lacking?
No training implemented. No training documentation. Nothing to indicate that Investigator has completed the Corrective Action. No ZE created. Drawing not corrected. No explanation / detail. Future tense. No training implemented. No training documentation. No ZE created. Open ended suggestions or recommendations are not considered corrective action. No action taken. No training. Written in future tense. This is a suggestion / recommendation, not a corrective action.

Part should not be used and Engineering should look at the drawing to correct it so it wont happen again.

At several stages, the XXX defect was missed. We will remind the Operators to be more careful. There may be a problem with the gages.

Recommend that a new test procedure be written and that the procedure will be alright with customer before release.
