EFFECT OF SANKARA SWEDA AND TRIVRUT CHURNA VIRECHANA IN
AMAVATA A CLINICAL STUDY

Amavata is commonest among chronic inflammatory joint diseases in which joints Become
swollen, painful, and stiff. It is a debilitating disease in view of its chronicity and
Complications. Therefore, it has taken the foremost place among the joint disorders. The
figures of prevalence vary substantially ranging from 0.3% to 1% of the population Indian
data suggests the prevalence to be around 0.65% to 0.75% of the population. 1-3% of women
may develop RA in their life time. Amavata is having Bahudoshatvastha, where Kapha, Vata
and pitta doshas are predominantly involved.
Its Udbhava sthana is Amashaya, Vyakta sthana is Sandhi. In Amavata mainly kapha sthana
like sandhi is affected. Langhana and Swedana is considered as the prime line treatments for
amavata. Virechana is considered under shodhana variety which helps in pratiloma gati of
vata. Here an attempt is made to evaluate the “Effect of Sankara
sweda and Virechana in Amavata”.
Objective of the Study:
1. Evaluation of Effect of Sankara sweda in Amavata.
2. Evaluation of Effect of Virechana in Amavata
3. Evaluation of combined effect of sankara sweda and virechana in amavata
Study Design:
It was a simple randomized single clinical observational study with a pre and post test design
in 30 patients, were diagnosed as Amavata and fulfilling the criteria of undergoing the
process of pinda sweda and virechana were selected. Patients were subjected to
deepana/pachana with Panchasama choorna 3-6 gms with hot water before food thrice daily
till amapachana. After amapachana, abhyanga with Brihat saindhavadi taila and karpasadi
bheeja choorna panda sweda was administered for 7 days. After one day rest, virechana with
Trivrut choorna mixed in kanji was given in
morning of next day. Patients were subjected to samsarjana karma based on their shuddhi
lakshanas. The assessment criteria were noted before, during and after samsarjana karma
followup.Subjective parameters i.e. Bahusandhishoola,
Bahusandhishota, Stabdhata and Spershasahishnuta are the chief complaints of Amavata and
objective parameters are Hb%, ESR,CRP, RA, NRS (Assessment of pain), VAS (Assessment
of pain), RAI, EAMRAI, GDA, AIMS, Physical Disability, Walking Time, Grip Strength,
Range of Movements, DAS criteria, Ayurvedic health assessment, Assessments are done
before and after the treatment.
Results:
• 100% patients manifested with agnideepthi, tandrahani, sandhi chesta and tvak prasada after
sankara sweda.
• The average time taken for initiating the first virechana vega is 105 minutes.
• 100% patients showed the symptoms like agnideepti, shariralaghuta and vatanuloma after
virechana.
• Marked reduction was seen in the Pain (49.98%), Stiffness (37.75%), tenderness (52.38%),
and swelling of the joints (55.15%).
• The assessment of the overall effect of the treatment revealed that 73% (22) of the patients
showed the Good Response to the treatment. Complete remission of total effect of therapy
shown in 7 %( 2) patients and Remaining 20 %( 6) patients responded to Moderate Effect.
