How to get the Guidelines and Haj Application Forms (HAF)

The Guidelines and Haj Application Form for each Cover is available free of cost with the concerned State Haj Committee or Download Application Form / Guidelines. from website www.hajcommittee.com. The downloaded HAF can be filled in directly on the computer and print out of filled up form can be taken.

How to fill the Form

General: Read Guidelines carefully before filling HAF. Fill all the columns of the HAF compulsorily. Fill the HAF (in duplicate) in English BLOCK letters. Furnish correct name and other information, as appearing in International Passport. Please note that incomplete HAF will not be accepted.

Medical screening and fitness certificate:

o o o o Medical screening and Fitness Certificate must be filled compulsorily by Doctor preferably MBBS/Govt. Doctor. Details of Blood Group/ Diabetic and Blood pressure must be written. Name of Doctor and Registration No. must be mentioned. Signature / Thumb Impression of Applicant is essential . Signature of Medical Practioner with complete address and Seal stamp is must.

Details of processing fee of Rs.300/- per Pilgrim deposited (Use pink slip attached with form)

Deposit Rs.300/- per Pilgrim towards the non refundable processing fee in the Haj Committee of India Bank Account No. 33564923057 maintained with the State Bank of India through Core banking system.

Procedure for sending the Filled Haj-2014 Application Form:

Fill all the fields of the Haj Application Form, take the print out on A4 page size paper (which appears on the screen after submitting the filled application.) The completed HAF must have the signature / thumb impression of the applicant, nominee and mehram ( for female Pilgrim). With the Haj Application form enclose the following documents in duplicate; Proof of address. Original passport / Copy of passport, as applicable Cancelled blank bank cheque / copy of first page of bank passbook / Certificate issued from bank. Pay-in-slip indicating payment of processing fee Solemn Declaration(s) on plain paper. Latest colour photographs of 3.5cm X 3.5cm size with white background front pose must be pasted at notified area on the Haj Application Form. Medical screening and fitness certificate duly filled in.

(If the HAF is submitted without the enclosures and documents, required as per the Guidelines, it shall be treated as incomplete and shall be summarily rejected.) Application form with all the enclosures listed should to sent through Registered/Speed Post /Courier/Hand delivery, so as to reach on or before dated 15.03.2014, 5:00 PM to the Executive Officer / Secretary of the concerned State Haj Committee (see the address of SHC Guidelines page no 23 & 24).
