GENERIC NAME: Acetaminophen

BRAND NAME: Tylenol

USUAL DOSE/ROUTE: Oral 325-640 mg (3-4 time daily) DRUG ACTION: Analgesic for mild/moderate pain as sole agent or in combination with other opiods and/or analgesics. Antipyretic. Does not possess anti inflammatory properties. USES: Mild to moderate pain or fever, arthralgia, dental pain, dysmennorhea, headache, migraine, myalgia, osteoarthiritis. POTENTIAL RISKS/SIDE EFFECTS: Uticaria, erythematous reactions, anemia, leukopenia, neutropenia, pancytopenia, jaundice, GI bleeding, seizures, rash, renal failure, nausea, vomiting, ab pain. NURSING IMPLICATIONS: Excessive use causes bleeding, watch use with alcohol, antileptic. Administer with food, OD= sweating, pallor, cyanosis, oliguria, avoid aspirin and salicylates. RELEVANT LAB/ASSESMENT VALUES AND DATA: AST, ALT, bilirubin, creatinine, BUN, urine creatinine, occult blood, albumin., CBC, and PT. OTC blood glucose may give false positive. USUAL DOSE/ROUTE: IV bolus: 80 mg, IV NF 18mg, INJ: 5,000 UNITS, DRUG ACTION: Prevents conversion of fibrinogen to fibrin and prothrombin to thrombin by enhancing inhibitory effects of antithrombin III, and therefore working as an anti coagulant. USES: PE, MI, A fib, prevention of deep vein thrombosis. POTENTIAL RISKS/SIDE EFFECTS: fever, chills, headache, hemorrhage, rash, alopecia, hyperkalemia, anaphylaxis, anemia, thrombocytopenia, hematuria, uticaria, hypoaldosteronism. NURSING IMPLICATIONS: May hold during menstration, report abnormal bleeding, take at same time each day, avoid anything that may induce bleeding, RELEVANT LAB/ASSESMENT VALUES AND DATA: aPtt, CBC, platelet count, HCT, PTT GENERIC NAME: Ondansetron BRAND NAME: Zofran

USUAL DOSE/ROUTE: .15 mg/kg or one 32 mg single dose. Hepatic dose: 8 mg/day.

DRUG ACTION: antiemtic, prevents nausea, vomiting by blocking serotonin peripherally, centrally, and in the small intestine. Uses: prevention of nausea and vomiting associated with cancer chemo, prevention of post op nausea and vomiting. POTENTIAL RISKS/SIDE EFFECTS: headache, dizziness, drowsiness, fatigue, fever, back pain, dry mouth, diarrhea, musculoskeletal pain, dry mouth, rash, bronchospasm, hypoxia, urinary retention. NURSING IMPLICATIONS: monitor ecg, assess therapeutic effect: no vomiting or nausea. Headaches are common, monitor for shuffeling gait and tremors. USUAL DOSE/ROUTE:GERD: PO 40 mg/day x 8 wk, may repeat course. Erosive esophagitis: IV 40 mg/day 7-10 days, PO 40 mg/day x 8 wk (may repeat PO course).: DRUG ACTION: suppresses gastric secretion by inhibiting hydrogen/potassium ATPase enzyme system in gastric parietal cell, gastric pump inhibitor: blocks final step of acid production. USES: GERD, erosive esophagitis. POTENTIAL RISKS/SIDE EFFECTS: Bronchitis, diarrhea, headache, dizziness, nausea, Arthritis, chest pain, dyspepsia, hearing loss, vertigo, vomiting, contact dermatitis, insomnia, fatigue, pneumonia, hyperglycemia, weight gain/loss, myalgia, ab pain. NURSING IMPLICATIONS: assess respiratory, Constipation is a common side effect and may be prevented with adequate fluid and fiber intake. Monitor for orthostatic hypotension. Take with food. Bowel sounds q8hr, avoid alcohol and nsaids. RELEVANT LAB/ASSESMENT VALUES AND DATA: hepatic and GI studies/labs. (AST,ALT)

GENERIC NAME: HydrALAzine

BRAND NAME: Apresoline

USUAL DOSE/ROUTE: 10 mg qid 2-4 days, then 25 mg for first week, then 50 mg qid individualized to desired response. DRUG ACTION: Vasodilates arteriolar smooth muscle by direct relaxation; reduction in blood pressure with reflex increases in heart rate, stroke volume, cardiac output. USE: essential hypertension and severe. Unlabeled: CHF.

POTENTIAL RISKS/SIDE EFFECTS: headache, tremors, dizziness, anxiety, depression, peripheral neuritis, depression, fever, chills, shock, orthostatic hypotension, nausea, vomiting, anorexia, diarrhea, leukopenia, thrombocytopenia, rash, uticaria, edema, muscle cramps, palpitations. NURSING IMPLICATIONS: Asses cardiac status and BP, weigh daily, I&Os, Edema in legs and feet, crackles and dyspnea, affects mood and mental status, IV site for extravasation, avoid OTC, rise slowly, take with food, PO route is usually preferred. RELEVANT LAB/ASSESMENT VALUES AND DATA: K, Na, Cl, CO2, CBC, serum glucose, LE prep lab values. Half life 3-7 hours.
