Gordons Assessment Health perception/ Health Management Pattern The patient was admitted due to epigastric pain.

She understands that the purpose of her treatment is to treat her illness. She was hospitalized before due to amoebiases, Diabetes Mellitus, burn injury on her foot and due to gallstones and diabetic retinopathy.

Nutritional/ Metabolic Pattern Before: The patient has good appetite, she usually eats meat most of the time and vegetables. She doesnt have any food allergies but has food restriction on pork due to her religious practices. She takes Iberet as her iron supplement. She always goes to fast food restaurants because she loves to eath there. She didnt experience any difficulty in eating and swallowing. She has dentures on both upper and lower part because she already had all her teeth extracted before. She usually consumes 4 glasses of water in a day.

During hospitalization: The patient has poor appetite. She usually eats am for her meals. She has food restrictions on fatty foods and those high in cholesterol. She has dentures on bother upper and lower part. She consumes 1-2 glasses of water daily

Elimination Pattern Before: The patient had her regular bowel movement of once or twice a day. She already experienced constipation and diarrhea before. She didnt have any problems before, during and after urination. She didnt experienced any skin problems and didnt have any odor problems Exercise and Activity Pattern Before: Her typical days activity is teaching because she is a teacher. She didnt have any regular exercise but sometimes she does walking for 30 minutes if she has vacant time. She didnt experienced chest pain and difficulty of breathing. She doesnt smoke. Sleep/ Rest Pattern Before: The patient usually arises at 6 oclock in the morning and sleeps at 10 oclock in the evening. She doesnt take naps. She feels rested after sleeping. She doesnt have any aids in sleeping and didnt experienced nightmares. She didnt experienced insomnia.

During: The patient defecates once or twice a day. She doesnt have any discomfort of difficulty before, during and after urination. She doesnt have any skin problems. She doesnt have any odor problems.

During: During hospitalization, her usual activity is lying on bed and watching television. She doesnt have any exercise.

During: The patient had interrupted sleeping pattern during hospitalization. She has difficulty sleeping due to the pain the right upper quadrant of her abdomen. She doesnt use any aids in sleeping

Cognitive and Perceptual Pattern Before: The patient had her last eye examination last year and was diagnosed of having Diabetic Retinopathy. She uses eyeglasses. She didnt experience any hearing problems and any unusual ear discharges she experienced pain on her abdomen.

During: The patient uses eyeglasses. She doesnt have any hearing problems. She experiences pain on her right upper quadrant of her abdomen

Self- Perception Before The patient feels good most of the time. She is contented with what she has. There is nothing about herself that she likes to change. She prays to Allah to alleviate her feelings. Role-Relationship Pattern Before She can easily get along with her family. She gets support from her family. She turns to her daughter for help Coping Stress Before The situation that caused stress in the past years was her separation from his husband. She usually solves her problems by the help of her family. She is able to handle her problems successfully most of the time Values/ Beliefs Before The most important thing to her is her family. She also puts her trust to Allah and let Him take over in everything. She finds prayer and meditation very helpful

During She feels bad about her condition. Her illness affects the way she feels about herself. She is anxious that her condition might get worse. She talks to her daughter and sister to alleviate her feelings. During She verbalized that her role in her family is still the same although she has health problems. She can still get along with her family. During She is having a hard time adjusting to her situation. This causes her stress. She turns to her children in times of crisis. She asks Allahs guidance always.

During The most important thing to her is her family and her health. Her sickness didnt affect her belief to Allah.
