SUMMARY Semarang City is located on flat and low topography in the north and the mountains region in the

south. Semarang City has severa rivers, densely populated areas and the land is lower than sea surface.During the rainy season , these conditions make one the cause of the flood. The impact of flooding are appear health problems and impaired quality of productivity the society. Based on the results of UNFCCC / COP 13th on 2007 in th Bali, climate change triggers rising infectious disease, one of them is Leptospirosis. International Leptospirosis Society on 2011 stated Indonesia leptopsirosis deaths value is high at 2.5 % -16 , 45 % and ranked third on the world. Based on the statement of Semarang city Health Office Head, Leptospirosis become the second disease after dengue hemorrhage fevr in Semarang city. Regional settlements with high population density, flood, high number of rats has the potential of disease Leptospirosis. Leptospirosis caused by Leptospira interrogans bacteria, found in the urine of animals infected with that bacteria. This disease transmitted to the humans through the urine of animals (cows, pigs, dogs and rats), water or soil contaminated with L. interrogans, then the bacteria are in direct contact with skin wounds, mucous membranes of the eyes, nose of humans. L. interrogans are aerobic , rod-shaped bacteria and can survive outside the body several weeks from the animal. The bacteria will be survive in the acidity of the soil, water neutral, temperature 28OC- 30 OC Based on data from Health pocketbook Central Java Provincial Health Office in 2012 and the Health Profile of Semarang City in 2012, leptospirosis cases and deaths in the Semarang City in 2008-2009 has increased , whereas in 2010 to 2011 cases decreased , but the increase in number died . In 2012 the increase in cases is 81 cases , a decrease in the death rate of 14 people. Statement of the Head of Communicable Disease Control and Prevention ( P2P ) Semarang City Health Office said early in January 2013 had recorded 5 cases , 2 people were dead. Based on data from Semarang city health department , the number of patients leptosirosis on November 2013 recorded 66 person. Leptospirosisi reduction efforts that have been did by the Health Department Semarang City is health education . However , Based on the results of the study indicate that the early detection of cases still passively by officers P2B2 Leptospirosis health Center of Semarang and wait for confirmation of the patient and other agencies with a percentage of 91.9 % and 40.5 % of the confirmed cases of hospital and Semarang City Health Office . 32.43 % . PHN program at the health center can not run well, because of several things. Example the more

prominent curative Health Center, Health Center nurses are still not doing the duties and functions well . High prevalence of leptospirosis due to 3 factors , namely triangle epidemiology, lack of public knowledge on leptospirosis the government's role in the prevention of leptospirosis less than optimal. PHN is a Health Center Program , which the program can be a solution to reduce the prevalence of leptospirosis. PHN has preventive and promotion services on an ongoing basis without neglecting curative and rehabilitative services, and there is an active partisipation of society. PHN target are individuals, families, groups and communities with priority. Nurses are very influential in the implementation of PHN because nurses have knowledge that can be applied in this program, it is called community nursing PHC nurses in running PHN concern on promotion and preventive. Promotive actions performed by KEPMENKES No.1193/MENKES/SK/X/2004, 18th October 2004 about National Health Promotion policy which has a core are Improved health education to the community, Development of health promotion media and communication technologies, Information, Education and Communication ( IEC), Development of Public Health Efforts Sourced (UKBM). Preventive, carried out by three levels of prevention. There are Level 1 : Promotion & Protection,Level 2 : Early Diagnose & Prompt treatment, Level 3 : Limitation Disability & Rehabilitation. In carrying out promotion and prevention , the nurse inserts the PHBs and provide the motivation to do so . PHBs are a form of community empowerment conscious , willing and able to use the knowledge , Sikan and proactive measures to maintain and prevent the risk of disease , protect yourself from the threat of disease
