REQUEST FOR INFORMATION - DETAILED INFORMATION

I note that you are now onshore, therefore in order to facilitate the assessment of this application, you are requested to provide the following additional information listed below: SKILLS AND QUALIFICATIONS Evidence of 900 Hours Work Experience Please submit evidence of your 900 hours work experience. The evidence you provide should include copies of all documentation submitted to the Trades Recognition Authority (TRA). If you have undertaken voluntary work experience, you are required to provide additional evidence of how you supported yourself nancially during this period of unpaid employment. Examples of documentation are:

Detailed work references showing duties and duration of employment, to be signed by your manager; Pay slips covering the period of claimed work experience showing Pay As You Go income tax deductions; Superannuation statements; Bank statements showing payment of salaries into personal bank accounts; Australian Taxation Ofce (ATO) assessments; Group Certicates; Timesheets or logbooks.

HEALTH REQUIREMENTS Undergoing the Health Assessment for your Visa You are required to undergo a health assessment(s) to determine if you meet the health requirement for the grant of an Australian temporary visa. Required medical examinations and x-rays are conducted in Australia by Medibank Health Solutions or an Approved Medical Practitioner (AMP) in a regional area, using paper or electronic forms as outlined below:

If you lodged your visa application electronically, then you can download forms for medical examinations (Form 26EH Medical examination for an Australian visa) and/or x-rays (Form 160EH Radiological report on chest x-ray of an applicant for an Australian visa) via the online Query Application Status service on our website www.immi.gov.au. To access the forms, you will need to provide your Transaction Reference Number.

When completing Form 26 Medical examination for an Australian visa and Form 160 Radiological report on chest x-ray of an applicant for an Australian visa please indicate that you intend to stay in Australia temporarily. Provisional applicants should indicate a permanent stay.

Please contact your nearest Medibank Health Solutions ofce to make a booking to undergo the health examinations that appear against your name on the enclosed Request Checklist. The preferred method of contacting Medibank Health Solutions is via their online booking service at www.medibankhealth.com.au. However you should telephone Medibank Health Solutions if you wish to arrange a family appointment or if you are in a country area to obtain the name of the AMP closest to you. Medibank Health Solutions can be contacted on 1300 361 046 or at www.medibankhealth.com.au. CHARACTER REQUIREMENTS Police Certicates Police clearance certicate for each applicant aged 16 years or over from India and every country where you have lived for a total of at least 12 months in the last 10 years. Please refer to the enclosed form Character requirements penal clearance certicates for further instructions. You must provide the original certicates. For information on obtaining police clearances please refer to the Character Requirements Penal Clearance Certicates available at http://www.immi.gov.au/allforms/character-requirements/index.htm Australian Federal Police Clearance If you have lived in Australia for a total of 12 months or more in the last 10 years (these 12 months are calculated cumulatively, not consecutively) you need to obtain an Australian Federal Police (AFP) clearance by completing a National Police Check Application Form. The form is available from the AFP website at http://www.afp.gov.au/what-we-do/police-checks/national-police-checks.aspx. Please send your completed form to the AFP at the address on the form. If you do not have access to the internet you can request for a form to be posted or faxed to you by calling the AFP helpdesk on +61 2 6202 3333. Form 80 Please update Form 80 (Personal Particulars for Character Checking) with your current details (employment, education and travel) and return immediately.
