PEARL Reflection Template Name of Experience/Event: Floor-ready patient rapidly progressing to the oscillator Type of PEARL: Clinical Ethical

Social Date of Experience/Event: November 2 !" #rief $e%cription of experience or event: I was about 3-months off of orientation, and saw I was getting report from a nurse who was 3 days off of orientation. The patient, a 2-year-old T&A with a genetic syndrome, had been ready to go to the floor and had suddenly had an acute desaturation episode within the last hour or two and was rapidly declining. hen I arri!ed, the team had "ust emergently intubated, was lining the child, and the off-going nurse loo#ed horrified and bewildered. Throughout my shift, the patient was started on pressors, put on the oscillator, and was e!entually stabili$ed and treated for an A%&' picture. Reflection abo&t experience or event 'Challen(in( a%pect%) *hat yo& $i$ *ell if applicable) *hat %&rpri%e$ yo&) *hat yo& learne$ from it) ho* it contrib&te$ to yo&r profe%%ional (ro*th) etc+, This was the first time I had come into a situation where the patient(s condition was acti!ely deteriorating, the room was a mess, and I had to be the nurse to )hopefully* clean things up. Though I was terrified wal#ing in there, I was technically the more +senior nurse, and wanted to wal# in with confidence and be helpful to the off-going nurse. e e!entually got through report, and then I hit the ground running. It was one of the busier nights I(!e had in the -I./, and there were se!eral ta#e away learning points that stuc# with me. 0ne is the importance of getting your lines situated first1 ma#e sure the offgoing nurse can show you what(s what, and if not, figure it out right away2 'econd, use your charge nurse and other resources in this situation. If charge wants to come in and +ta#e o!er, for a few minutes, let her, and be humbled by it. 3!en if it feels li#e you are relin4uishing control and you thin# she(s doing it because she doesn(t thin# you #now what you(re doing, ta#e what you can from it. 5ecause she #nows so much more than you, and that(s o#2 I learned so much that night about how to +set up, the en!ironment in the room of a really sic# #id. Finally, don(t be afraid to ha!e others tal# to your #id(s parents for you, or be honest with them that you really can(t de!ote the time you would li#e to answering all of their 4uestions during a critical moment. I was stressing that night about trying to tal# to the parents while doing clinical tas#s that, as a new nurse, needed my undi!ided attention. In future situations I was more able to say, +%ight now I(m focusing on doing 6666 because we(re worried about 666, and I(ll be more able to answer your 4uestions once I(m done., 0!erall, this was definitely an assignment that left an impression on me and strengthened my ability to cope with and care for the sic#er child.
