APPLICATION FORM FOR ADMISSION INTO DIPLOMA AND CERTIFICATE
PROGRAMMES FOR THE ACADEMIC YEAR 2014/15

NOTES
a) Complete the application form using capital letters
b) Application forms must be accompanied with a copy of Bank pay-in slip non
refundable Tsh. 20, 000/= as application fee. List of agricultural training institutes
and respective bank accounts is attached for reference.
c) All application forms must be duly filled, signed, attached with copies of birth
certificate, academic certificates and transcripts.
d) Applicants must meet all entrance requirements of the programme otherwise,
applications shall not be processed.
e) Employed applicants should attach release letter from their employers
f) Applicants should post the application form direct to the respective institute not later
than 30
th
June 2014. List of agricultural training institutes and their respective
postal address is attached for reference.
g) Successful candidates shall be informed through their postal address or Website:

h) Qualifications
1) Certificate Course
Certificate in General Agriculture (NTA level 5): applicant must have at
least four (4) passes of grade D in science subjects (Physics, Chemistry,

Biology, Geography, Mathematics and Agriculture) in the Ordinary Certificate
of Secondary Education. English will be an added advantage.
2) Diploma Courses
i. Diploma in Land Use Planning: applicant must have Advanced Certificate
of Secondary Education with at least two (2) subsidiary passes in science
subjects (Physics, Chemistry, Biology, Geography, Mathematics, and
Agriculture). OR Applicant must have passed Certificate in General
Agriculture (NTA level 5) and must have passed at least four (4) science
subjects (Physics, Chemistry, Biology, Geography, Mathematics and
Agriculture) with grade D in the Ordinary Certificate of Secondary
Education.
ii. Diploma in Irrigation: applicant must have Advanced Certificate of
Secondary Education with at least two (2) subsidiary passes in science
subjects (Physics, Chemistry, Biology, Geography, Mathematics, and
Economics). OR Applicant must have passed Certificate in General
Agriculture (NTA level 5) and must have passed at least four (4) science
subjects (Physics, Chemistry, Biology, Geography, Mathematics and
Agriculture) with grade D in the Ordinary Certificate of Secondary
Education.
iii. Diploma in General Agriculture (NTA Level 6): Applicant must have
passed Certificate in General Agriculture (NTA level 5) and must have
passed at least four (4) science subjects (Physics, Chemistry, Biology,
Geography, Mathematics and Agriculture) with grade D in the Ordinary
Certificate of Secondary Education.
iv. Diploma in Food Production and Nutrition: applicant must have Advanced
Certificate of Secondary Education with at least two (2) subsidiary passes in
science subjects (Physics, Chemistry, Biology, Geography, Nutrition, and
Agriculture). OR Applicant must have passed Certificate in General
Agriculture (NTA level 5) and must have passed at least four (4) science
subjects (Physics, Chemistry, Biology, Geography, Mathematics and

Agriculture) with grade D in the Ordinary Certificate of Secondary
Education.
v. Diploma in Agro mechanization: applicant must have Advanced Certificate
of Secondary Education with at least two (2) subsidiary passes in science
subjects (Physics, Chemistry, Geography, Mathematics, Economics and
Agriculture). OR Applicant must have passed Certificate in General
Agriculture (NTA level 5) and must have passed at least four (4) science
subjects (Physics, Chemistry, Biology, Geography, Mathematics and
Agriculture) with grade D in the Ordinary Certificate of Secondary
Education.
vi. Diploma in Horticulture: applicant must have Advanced Certificate of
Secondary Education with at least two (2) subsidiary passes in science
subjects (Physics, Chemistry, Biology, Geography, Nutrition, and
Agriculture). OR Applicant must have passed Certificate in General
Agriculture (NTA level 5) and must have passed at least four (4) science
subjects (Physics, Chemistry, Biology, Geography, Mathematics and
Agriculture) with grade D in the Ordinary Certificate of Secondary
Education.
vii. Diploma in Crop Production: applicant must have Advanced Certificate of
Secondary Education with at least two (2) subsidiary passes in science
subjects (Physics, Chemistry, Biology, Geography, and Agriculture). OR
Applicant must have passed Certificate in General Agriculture (NTA level 5)
and must have passed at least four (4) science subjects (Physics, Chemistry,
Biology, Geography, Mathematics and Agriculture) with grade D in the
Ordinary Certificate of Secondary Education.
