INSTRUCTIONS

General:
1. Students are advised to go through the complete instructions carefully before filling the application form.
2. Download the form and take the printout without changing the size.
3. All re-sitting students must use only this form. Re-sitting forms issued for previous examinations will not
be considered.
4. Use bold capital letters while filling the application form.
5. Please note that incorrect filling, overwriting, cutting and incomplete application will be rejected.
Therefore, ensure that the form is filled with utmost care for accuracy and clarity.
6. Use only Black ink or Navy blue ink ballpoint pens for filling the application.
7. The students required to appear for project internal papers (such as MT0010, MT0021, MT0025, MT0029,
etc) should also submit the re-sitting application form along with the requisite fee.
8. No student is permitted to appear for more than one examination at a scheduled time
9. Wherever students apply for re-sitting papers that clash with other papers in the schedule of the
examinations, admit card will be issued for all such papers. However, the student is permitted to appear in
any one of the papers of his choice.

Important:
1. Column 1 & 5: Roll Number and Name shall be as per Registration documents of the university.
2. Column 6: Provide complete address with pin code.
3. Column 11: Select appropriate paper code from the enclosed Table. Special care should be taken to use correct
code number. Subjects having two credits combined together and administered as one four credit paper, are now
removed. Henceforth, two credit paper examinations will be conducted separately.

Procedure for filling up University Exam, Internal Assessment Columns:
(a) If the student is applying for only internal part then he/she should specifically mention "YES" under
"Internal Assessment (IA)" column.
(b) If the student is applying for only University part then he/she should specifically mention "YES" under
"University Examination (UE)" column.
(c) If the student is applying for both University and Internal part then he/she should specifically mention
"YES" under "UE/IA" column.
Note: Please be very careful in mentioning the paper code and the subject title. In case of mismatch, the paper code
alone will be considered for issuing Admit Card.
4. Column 12: Provide the Demand draft details clearly in the appropriate columns.

5. Payment should be made in the form of DD drawn in favour of "Sikkim Manipal University, DE" payable at
Manipal / Udupi and students should endorse their Name, Roll Number and Centre Code on the reverse of
the DD. Payments made other than in the above form will be at own risk. The fees once paid cannot be
refunded.
6. Forward only the front page of the re-sitting application duly completed along with the demand draft.
7. The learning centre and students are also advised to retain a photocopy of the filled application and the draft for
future reference, if any.
